Employee #1 was standing near Employee #2, who was using a hammer to remove a bearing from the manlift. Employee #1 suddenly felt pain in EE's wrist and, upon lifting EE's sleeve, observed a small cut. EE was transported to Excel ER, where an X-ray revealed a small piece of metal embedded in EE's right wrist.

An employee was using a 4-inch angle grinder when debris went underneath EE's safety glasses and irritated EE's right eye. The employee stated EE could not use face shield at the time because EE was in very tight quarters while grinding. The employee was taken to E.R., where EE's eye was flushed and the debris removed. EE was given a full release with no restrictions.

Employee overfilled the Diesel Exhaust Fluid tank (DEF), causing it to spill on their clothing & glasses. A drop of DEF entered left eye. They immediately flushed with water, then changed into clean clothes. They requested a medical evaluation to ensure eye was okay. They were taken to ER, where they were examined & medically cleared to return to work without restrictions.

Employee cleaning under tail pulley of HSI motor, saw a hanging drive belt, informed crew. While assisting with belt replacement, 2nd employee cut belt while 1st employee's hand was under drive motor. Drive motor held in place by belt & a pivoting bracket. Once belt was cut, motor pivoted, landing on employee's hand, smashing it. The employee was taken to ER for treatment.

Employee fractured right leg while helping load equipment onto a low boy trailer. They stepped into a hole where a wood plank was missing. They fell off the trailer and their foot stayed in the hole.

Employee was removing small rocks from feeder when larger rock broke loose. It fell and hit EE's outer left ankle.
