Cleaning under Rotex area. Backing up with Dingo when left wrist got pinned under structure beam and dingo controls.

Employee was removing organics from screener when EE noticed a slight pain in EE's back from twisting and pulling. Later the employee was feeling more pain while operating a loader.

Employee used leverage, via pushing by hand, to break a 4.5" PVC pipe, upon breaking the pipe flipped up and struck the employee in the mouth causing a laceration on the lower lip.

EE was entering the skidsteer when EE's phone fell out of EE's pocket. It fell on the bucket step into a corner. EE sat down and went to pick it up quickly and when EE pulled hand up noticed the left index finger was bleeding. When EE flexed the finger EE noticed the severity of the cut and it started bleeding profusely.

Employee was opening the lid on a pneumatic truck. The lid on the handle was tight and employee exerted a good amount of force to get it to open and felt something pull in shoulder muscle. Employee called and said employee wanted to take pain medication and see if it felt better. After about an hour the pain was not getting better and we decided to take employee to get checked out.

Employee was closing lids at our loadout facility and was not maintaining 100% tie off while on the customer truck. The truck pulled forward and the employee fell 15 feet to the concrete floor.

Employee got sand in EE's right eye while performing housekeeping duties. Was treated onsite with first aid measures, then taken to an offsite facility for evaluation where EE received an eye drop prescription.

Employee was operating a skid steer around a screening location. The following morning the employee complained of facial irritation. Employee was taken to Quality Urgent Care in Pleasanton, TX for evaluation. Diagnosis was Dermatitis to EE's face. This event was reported more than 24 hours after being present at work.

While operating a skidsteer the equipment overheated. The employee exited the equipment to inspect the issue and while the engine compartment was open the radiator overflow line had whipped up and a burst of hot coolant came out hitting their hands. The overflow hose was not secured in place due to the mounting brackets being on the wrong side of the radiator frame.

Employee was going to wash off grease fittings. The employee was having an issue with the hose, so the operator was going to dismount the loader to help. The operator did not realize the loader was still in gear and as operator stepped off the brake pedal, the loader moved forward. The front right tire bumped the employee's knee.

Weather was sunny and damp. Employee was replacing springs on the duster in the dry plant when a metal plate slipped and smashed EE's hand between the metal plate and the metal structure.

Employee was backing up a dingo pushing dand when EE's hand was pinched in between the dingo controls and a cross member for the duster. There was light swelling and redness, treated as a first aid incident. When irritation persisted it was learned that the wrist was broken.

The employee added oil a machine and then climbed into the cab to turn it off. After turning off the machine EE proceeded down the ladder of the machine slipping on the second rung falling approximately 4.5 feet to the ground landing on EE's buttocks. When EE got up EE felt a sharp pain in EE's pelvis and lower back causing EE to call for help as EE was alone.

The employee was removing a piece of equipment from the forklift to be worked on. The forklift was placed in a holding position but the emergency brake was not on and the controls were not touched when the forklift jerked forward striking the employee on the right side of body causing them to land on the connex box several feet away.
