Struck by... (Not Elsewhere Classified)

Employee was assisting moving a 50ft. section of poly pipe when it got hung-up on something. Once it was free it ""popped"", and the pipe struck employee on right knee and right hand.

Over-exertion (Not Elsewhere Classified)

Employee slipped in a muddy area however did not fall but sometime later felt some back pain. On 7/1/2024 employee reported this to the manager, and EE was seen at the Med Clinic for a lower back strain.

Employee was changing cutting edges on 988 loader and placed left hand on bolt head which cut palm of hand when tightened.

Caught in, under or between a moving and a stationary object

Employee was working on a VSI top plate lid and was lowering it when it lost pressure pinching employee's hand between the outer housing of the hydraulic cylinder and lower bushing. A loader was used to raise it back up and free employee's hand. The employee was taken by vehicle to medical center.

Employee was dismounting excavator and lost footing and twisted right ankle. EE has been continuing to work regularly and has had only observation type medical appointments, however on 4/11/2022 EE was given an injection and has been taken off work for 2 weeks as prescribed.

Over-exertion in lifting objects

Injured worker was working on 1/14/2020 when we reported that EE had injured self the previous day while changing crusher shoes. EE stated that EE pain in EE's chest when EE moved or went to pick something up and EE would have to stop and take a minute until the pain subsided. As soon as this was reported EE was taken to the clinic to be evaluated.

Absorption of radiations, caustics, toxic and noxious substances

Injured worker was eating lunch and felt a poking EE's leg and when EE looked at the bite area it was red and itchy. The next day the area was red and swollen and painful, so EE was taken to Dr. to be checked out. Worker was treated for insect bite and prescribed medication and returned to work.

Struck by flying object

Injured employee was utilizing a sledge hammer to remove teeth from the excavator bucket when a piece of metal broke off striking and puncturing employee right above the left knee.
