Employee put hand inside of guard for a drill, glove hit the drill bit and pulled hand into the drill bit injuring left ring finger.

Slip and Fall

Slip and fall, fell on a metal dump box, fractured jaw

Slip and fall on ice, cut knee on stone block

Slip & fall on cement floor, hairline fracture to right heel.

Employee was climbing on a series of power rolls to change a light bulb. The employee mis stepped falling through the rolls. Possibly fracturing his right femur

EE suffered a severe finger inury at 2:30 pm while operating a barrel trimming machine. He wipe away debris from the machine was operating to wipe the tile clean of debris. He failed to turn the rotary blade off. Blade struck his left ring and middle fingers. removal of approx. half inch of the left ring finger and approx. one inch of the left middle finger.

EMPLOYEE HIT FINGER WITH SLEDGE HAMMER.

WHILE CLEAVING A STONE, THE HAND HELD HAMMER FELL AND HIT THE EMPLOYEES LEG. EMPLOYEE NOTIFIED THE COMPANY THE LEG WAS BROKE.
