Miner demonstrated a standard threshold shift during hearing testing conducted on 2/5/2026.

DEBRIS FELL INTO EE'S RIGHT EYE WHEN REMOVING AN ITEM FROM AN UPPER SHELF. SUFFERED ABRASIONS TO THE EYE.

EE was shoveling around the plant for several hours. Felt discomfort later in the day in left shoulder area. Initially self-treated with OTC anti-inflammatories. Sought medical advice when pain and discomfort was not subsiding. Started physical therapy on 3/29/2021 for possible pinched nerve.

EE was cutting a piece of steel with a grinder when debris from grinding lodged in the right eye.

Employee was helping to lift the catwalk in the storage building. Employee felt pain in left shoulder.

Employee was stepping over a curve under the plant, slipped and twisted left ankle.

Loosening bolt on transmission, bolt let loose and right hand hit side of fender. Laceration to outside of right hand. (Per phone call he was using a wrench, working on a dozer. Laceration to index finger on right hand.)

Standard Threshold Shift per Annual Hearing Test.

Employee was lifting drill steel and strained left shoulder.

Employee was standing next to a wire trough that sits approximately 4"" from the ground. As he was turning to walk, he caught his foot under the trough. This caused him to fall to the concrete.

While installing conduit, a fabricator fell approximately 6ft when the board he was standing on shifted. While the fabricator initially reported no injury, the next day he felt soreness in his back and was sent to the occupational health clinic. He was found to have severely pulled some muscles in his lower back. He was subsequently placed on light duty and returned to work.

EE WAS INVESTIGATING WHY DUMP TRUCK TAILGATE WAS NOT OPENING. EE TRIED TO REMOVE THE AIR CYLINDER BY HAND. THE CYLINDER ACTIVATED WITHOUT ENGAGEMENT PINNING HIS RIGHT HAND BETWEEN THE CYLINDER AND THE TRUCK BODY.

EE was climbing a ladder, slipped and fell to the concrete.

Employee was working on a Screw Conveyor when he felt pain in groin area. Did not seek any medical attention at this time. In January he felt it should be looked at and was told he had a hernia.

SHOVELING SNOW & HEAVYLIFTING, EE HAS TENDINITIS IN LEFT ELBOW & HERNIA.

EE WAS OPERATOR LOADER, NOTICED PAIN IN LOWER BACK AND LEFT LEG WHEN HE EXITED LOADER.
