EE SLIPPED WHEN EXITING FROM EE'S PERSONAL VEHICLE UPON ARRIVAL AT THE OFFICE. SUFFERED A LACERATION TO THE SCALP AND BRAIN BLEEDS WERE DETECTED WITH A BRAIN SCAN.

While descending a ladder, employee slipped and struck rib cage against the ladder. Employee initially refused medical attention, but decided to get seen on 2/1/2023. Doctor performed x-rays on ribs/abdomen and confirmed there was no breaks, only bruising. Doctor note states employee can return to work with no limitations on 2/6/2023.
