EE SLIPPED WHEN EXITING FROM EE'S PERSONAL VEHICLE UPON ARRIVAL AT THE OFFICE. SUFFERED A LACERATION TO THE SCALP AND BRAIN BLEEDS WERE DETECTED WITH A BRAIN SCAN.

Miner demonstrated a standard threshold shift during hearing testing conducted on 1/24/24.

While descending a ladder, employee slipped and struck rib cage against the ladder. Employee initially refused medical attention, but decided to get seen on 2/1/2023. Doctor performed x-rays on ribs/abdomen and confirmed there was no breaks, only bruising. Doctor note states employee can return to work with no limitations on 2/6/2023.

EE was in #1 screen house sealing up cracks with caulking. EE had just finished the task and was on way back out when EE misjudged a step and fell forward landing on the catwalk edge cutting open left leg.

Employee was walking out to start loader for the day when employee slipped and fell on left arm.

While the employee was working on the bin feeder, they tripped, falling backwards and hit their right elbow.

WOOD HANDLE ON WHEELBARROW WAS SPLINTERED.

Employee fell and bruised his lower back and left side.

HEARING LOSS AT SOME HIGH FREQUENCIES PER ANNUAL HEARING TESTING REPORT.

The employee was working on belting. He noticed swelling in his knee.

While employee was working in the quarry, he tripped and twisted his knee.

Employee had difficulty in breathing, then was experiencing severe chest pain.

Employee cut his right index finger descending ladder on 777 Cat Truck.

NOISE INDUCED HEARING LOSS

Employee was cleaning out the back of his trailer with a shovel. His hand slipped off the shovel and the handle of the shovel jammed into his right side ribs.

EE was standing on catwalk of 350 Excavator hooking up battery charger to the excavator. Battery charger started to slip off catwalk and while EE tried to reach for the charger, he slipped and fell off catwalk to the ground.

Employee jumped up on passenger side of the truck to clean the window, he missed the step, fell back and when he landed on the ground, he twisted his left knee.

Walking from office to the loader, slipped on ice and fell. Fell on right leg and hip, hit head. Injury is described as Spinal Concussion.

Employee was using a sledge hammer to hit the bin

While performing his day to day tasks (Quarry Foreman) he has been experiencing constant pain in his right elbow.

EE WAS TRYING TO GET FROZEN STONE OUT OF BIN WITH SHOVEL.

HE WAS OPERATING LOADER AND CLAIMS HE HIT A BUMP IN THE ROAD WHICH CAUSED SEVERE BACK PAIN.

HE HAS EXPERIENCED A CONSTANT PAIN IN HIS LEFT ELBOW WHILE PERFORMING HIS DAY-TO-DAY TASKS. HE HAD EXPERIENCED NUMBNESS IN HIS TWO FINGERS ON LEFT HAND FOR ABOUT 4 OR 5 YEARS AND ABOUT A MONT H AGO THE CONSTANT PAIN IN LEFT ELBOW HAS BEEN OCCURRING.

EMPLOYEE WAS CARRYING EMPTY BOXES BACK TO THE EXPLOSIVES TRAILER, WHEN HE SLIPPED ON THE ICE AND FELL ON HIS HIP AND BACK.
