Mining near a fault line, the intersection of #3 Entry fell approximately 10 to 12 feet high. There was no damage to ventilation or equipment on the section. No miners were involved in the fall, so therefore no injuries.
A piece of rock fell from the roof of the mine mashing employee's hand between it and the drill pot of the machine.
EE reported Wednesday 1/3/07 by telephone to mine operator that on monday he had tripped on miner cable and injured his back.
The fall was in the return at 55217. Out by #2 drive in the return.
Employee lifted the cable reel to the TD2-43 roof bolter and strained his back.

EE was trying to remove a large chunk of coal on the belt line when he felt his back give out.
EE was operating the td2-43 roof bolter when he was struck in the finger with a thin sharp peace of metal.
A rock fall was discovered during the weekly examination of the return area. The fall was 3500 feet from the working section. The fall was at SS 550 approaching the #3 seal. The area will be dangered off and a walkway to the #3 seal will be timbered off for safe access to the #3 seal.

Bolter operator was lowering drill head when steel suddenly came loose and struck him. A piece of rock fell with the steel. Operator was struck.

HAD A ROOF FALL ON INTAKE 2 BRAKES OUT BY SPAD 1658 #2 ENTRY THE FALL CONTINUED FRAME OLD FALL, THE FALL SIZE IS 40 FT LONG, 18 FT WIDE TOP OUT AT 8 FT HIGH. MOVED LIFE LIME AND ESCAPE WAY TWO #1 ENTRY.
ROCK SLID OFF SIDE OF MINER ABOUT 7' HIGH IN INTERSECTION. FROM THE SIDE OF THE MINER IT SLID DOWN THE LEG OF THE INJURED WHO WAS PUTTING OIL INTHE MINER, CUTTING HIS LEG ABOUT 5". ROCK WAS 1' IN DIAMETER, 1/2 TO 1' THICK.
ROCK 18" TRIANGLE 18"X 18", 2" THICK ON ONE END TO 0" OTHER END. FELL ABOUT 3' AND STRUCK IN BACK BETWEEN SHOULDERS CAUSING BRUISE WHILE SHOVELING BELT.
ROOF FALL #1 ENTRY 1 XCUT INBY #55 1627. DANGERED AND TIMBERED OFF.
#2 ENTRY AT PORTAL 50' OUTBY 22 157. REMOVED ROCK OFF APRON, REWORKED AND EXTENDED APRON TOWARD INSIDE BRACKED WITH STEEL POST.
EE WAS TIGHTENING ROOF BOLT WITH WRENCH WHEN ROCK FELL AND HIT HIS LITTLE FINGER ON HIS LEFT HAND.
ROCK FALL BEING 20' WIDE, 25' LONG, 8' THICK OCCURED AT #3 HEADING.
