EE WAS CLEANING ROCKS FROM TAIL PULLEY USING HANDS INSTEAD OF A SHOVEL. THE BELT ROLLED BACK ONCE THE CLOG WAS CLEARED CAUSING EE TO PINCH HAND & ARM BETWEEN THE CONVEYOR AND THE HEAD PULLEY.
