Accident occurred on 07/25/2011; injured employee continued to work until 08/02/2011 when he went to the doctor and was excused from work on 08/03/2011.

EE slipped and fell inside of drill causing injury to his lower back and pelvis.

Caught in, under or between a moving and a stationary object

EE placed his right hand on the tailgate to bend over to see what was keeping the tailgate from closing. At this time, the tailgate closed, smashing his right middle finger causing a laceration that required suturing.

While pushing coal in stock-pile he stated he backed into the highwall injuring his lower back. IME report 12-23-2009 states that the lower back complaints are not causally related to the injury date,but were preexisting and naturally occurring such as a degenerative change or preexisting conditions.

Injured employee was loading galvanised pipe when he cut the palm of his left hand on the sharp edge of the pipe. Employee received 5 sutures.

Struck against a moving object

Personal Dodge Dakota pick-up truck owed and driven by other EE, as he was passing a coal truck he hit a rock on the berm,traveling 100' hit the second rock, traveling another 100' hitting the bank and third rock,causing pick-up to flip. resulting in laceration to his right index finger and passenger recieved facial fracture with laceration and small skull facture

As he was dismounting the dozer he tripped, fell foward and caught himself with the grabrail, twisting his left wrist. He was removed from work, due to swelling in his wrist.

EE WAS IN LINE TO DUMP AT THE BY-PASS WHEN ANOTHER TRUCK EXITING THE DUMP CLIPPED THE FRONT OF EE'S TRUCK, RESULTING IN EE HITTING HIS KNEE AGAINST THE STEERING WHEEL.

MEMBER WAS IN THE PROCESS OF ATTACHING A 6"" DISCHARGE HOSE TO A DIESEL PUMP. HE STEPPED IN A MUD HOLE, LOST HIS BALANCE AND FELL. MEMBER FELL ON PIECE OF BROKEN STEM, PIERCING AND CUTTING HIS RIGHT EAR.

HE WAS TRANSPORTED TO THE HOSPITAL WHERE IT WAS DETERMINED THAT HE HAD RECEIVED SECOND & THIRD DEGREE BURNS TO 20% OF HIS BODY.

GRINDING PAINT OFF TRACK PAD AND RAILS FOR REASSEMBLY, FOREIGN BODY ENTERED LEFT EYE, WOULD NOT WASH OUT.

EMPLOYEE WAS TIGHTENING A BARREL ON #2 SLURRY PUMP USING A BOXED END WRENCH. THE WRENCH SLIPPED OFF THE BARRELL AND HE FELL TO THE GROUND AND TWISTED HIS LEFT KNEE. HE SAW A SPECIALIST ON 8/28 AND IS STARTING PHYSICAL THERAPY 3 TIMES A WEEK FOR 3 WEEKS.

EE WAS GREASING AND CHECKING PUMPS, HE STEPPED ITNOT HE BASEMENT THROUGH FOR THE CLEANING PUMP AND TWISTED HIS BACK A PIECE OF FLOOR GRATING WAS OUT OF PLACE AND WAS INT HE PROCESS OF BEING RE PLACED WITH NEW PUNCH PLATE. EE DIDN'T RETURN TO WORK ON 8-4-00 BECAUSE OF MEDICATION PRESCRIPBED TO HIM.
