I NAME WAS CHIPPING SLAG FROM A WELD A PIECE OF SLAG BROKE OFF AND ENTERED HIS EYE HE WAS WEARING HIS SHIELD AND HAD A PAIR OF PERSCITION GLASSES ON UNDER THE SHIELD THE SLAG ENTERED FROM THE SHILED HE WAS WELDING ABOVE THE HEAD.

A DRILL STEEL W5S STUCK IN A DRILL HOLE I.NAME WAS TRYING TO DISLODGE THE STEEL WHICH WAS OFF CENTER THE DRILL HAD TO BE MOVED BACK AND JACKED UP ON RT SIDE I.NAME WAS ATTEMPTING TO LOWER THE KELLEY BLOCK D9IVE INTO THE TURN TABLE THE TURN TABLE WAS STILL TURNING I.NAME TRIED TO STOP THE TABLE AND CAUGHT HIS FINGER ON THE RT HAND UNDER THE TURN TABLE
