THE LOADER IDLED FORWARD TOWARD THE END OF THE LOADING RAMP. WHEN THE FOREMAN REALIZED THE LOADER WAS MOVING, HE JUMPED FROM THE LOADER ONTO THE ROAD AND BROKE HIS LEFT LEG ABOVE THE ANKLE.

MAINTENANCE CREW HAD CHANGED A VALVE CHEST ON A DUAL BOOM ROOF BOLTER AND THEY MUST HAVE GOTTEN SOME HOSES CROSSED UP. WHEN THE MACHINE WAS STARTED UP, THE DRILL STATION SUDDENLY SWUNG OUT AND PINNED THE INJURED MAN AGAINST THE RIB.

INJURED WAS SHOWING ANOTHER MAN WHERE SERVICE POINTS WERE LOCATED ON CONTINUOUS MINER. HE HAD RAISED A SHIELD TO SHOW SERVICE POINT AND WHEN HE WAS LOWERING THE SHIELD HE DROPPED IT, CATCHING HIS OWN FINGER.

EE WAS RIDING ON PERSONNEL CARRIER W/FOREMAN. WHILE BACKING THE RIDE UP IN A LOW PLACE THE REAR SHIELD CAME IN CONTACT W/MINE ROOF, SHEARING OFF THE BOLTS THAT HELD SHIELD DOWN. THE SHIELD CAM E AROUND & PINNED HIM BETWEEN THE SHIELD & BODY OF THE CARRIER.
