No 7 Entry 12 by 18 - 3 1/2 to 4 ft high. Roof was working on 8-1 Build 9 - cribs - dangered off and drop no 7 entry.

A roof fall occurred in No. 5 entry inby last open crosscut. The roof began ""working"" at about 8 AM - finally fell early afternoon. No equipment or people involved.

Injured was insorting roof bolt into previously drilled hole, when he slipped and his finger was caught between roof bolt plate and head of machine. Lacerating the small finger

A roof fall occurred at the mouth of no. 5 return entry at the surface. The roof fall measured 16 ft wide, about 7 ft long, and about 7 ft high.

Was crawling out of scoop deck, turning his body to crawl away from the scoop when he twisted his knee. EE refused first aid, was asked if he wanted an ambulance or brought outside, refused both.

THE INJURED WAS RIDING PERSONNEL CARRIER WITH LEG PROTRUDING FROM THE SIDE OF CARRIER. THE TOE OF HIS BOOT WAS CAUGHT ON MATERIAL ALONGSIDE THE ROADWAY, BENDING THE LEG BACKWARDS.

AN UNINTENTIONAL ROOF FALL HAS OCCURRED ON MAINS SECTION AT SPAD #461M #7 ENTRY. ROOF FALL WAS CAUSED BY A UNDETECTABLE SLIP. (HORSEBACK)

Fall from machine

THE LOADER IDLED FORWARD TOWARD THE END OF THE LOADING RAMP. WHEN THE FOREMAN REALIZED THE LOADER WAS MOVING, HE JUMPED FROM THE LOADER ONTO THE ROAD AND BROKE HIS LEFT LEG ABOVE THE ANKLE.

INJURED MAN WAS INSTALLING ROOF BOLTS, WHILE PUTTING UP A BOLT THE BOLT BENT, CATCHING HAND BETWEEN BOLT & DRILL POT.

INJURED PERSON WAS ROOF BOLTING AND DRILL STEEL WAS CAUGHT IN ROOF. HE WAS PULLING DOWN ON STEEL WITH ROTATION ON AND ENTANGLED HIS GLOVE WITH STEEL.

INJURED MAN WAS LOADING 6"" HOLLOW CINDER BLOCKS ON SCOOP WHEN HE TWISTED HIS BACK.

AN UNINTENTIONAL ROOF FALL HAS OCCURED ON MAINS SECT.2 BRKS OUT BY SPAD 300- #7 ENTRY.

MAINTENANCE CREW HAD CHANGED A VALVE CHEST ON A DUAL BOOM ROOF BOLTER AND THEY MUST HAVE GOTTEN SOME HOSES CROSSED UP. WHEN THE MACHINE WAS STARTED UP, THE DRILL STATION SUDDENLY SWUNG OUT AND PINNED THE INJURED MAN AGAINST THE RIB.

INJURED WAS SHOWING ANOTHER MAN WHERE SERVICE POINTS WERE LOCATED ON CONTINUOUS MINER. HE HAD RAISED A SHIELD TO SHOW SERVICE POINT AND WHEN HE WAS LOWERING THE SHIELD HE DROPPED IT, CATCHING HIS OWN FINGER.

EE WAS RIDING ON PERSONNEL CARRIER W/FOREMAN. WHILE BACKING THE RIDE UP IN A LOW PLACE THE REAR SHIELD CAME IN CONTACT W/MINE ROOF, SHEARING OFF THE BOLTS THAT HELD SHIELD DOWN. THE SHIELD CAM E AROUND & PINNED HIM BETWEEN THE SHIELD & BODY OF THE CARRIER.
