AREA WA S BOLTED WITH 4' FULLY GROUTED BOTLS ON 4'X4' PATTERN. NO APPARENT REASON FOR ROOF FAILURE. NO EFFECT ON VENTILATION. NO PROPERTY DAMAGES OR PERSONAL INJURIES. AIRCOURSE PASSABLE.

INJURED HAD HAND LAYED ON TOP OF ROOF BOLTER, WHEN A PIECE OF SLATE FELL STRIKING HAND.
PULLING GREASE HOSE THROUGH BELT HEAD. GREASE HOSE GOT CAUGHT IN HEAD PULLING HOSE AND INJURED ARM THROUGH BELT HEAD.
ON 11-30-93 EE MISSED WORK CONSIDERED OFF ON SICKNESS ACCIDENT EE ALLEGED A BACK INJURY COMPENSATION AWARDED ON 3-12-93 TO EE LONG BRONCHIA WAS NOT AWARE OF THIS UNTIL 4-30-93. LONG BRATCH 13A FELT THAT THIS WAS NOT REPORTABLE BECAUSE THEY WERE NOT AWARE THAT WAS MINE RELATED ACCIDENT LONG BRANCH 13A ONLY FILINING THIS FOR COMPENSATION.
