While moving some old hoses , one end came around and struck injury in the mouth causing a laceration. He received 4 stitches in his lower lip.

Injury stated that he was exiting the grader cab when his foot slipped and his arm and hand was still on the handrail which in turn, pulled his shoulder muscles.

EE was exiting the mechanic truck bed, when his foot slipped, causing him to strike his mouth on a hydraulic hose.

EE was running the grader in the members parking lot, when he saw another member fall in the parking lot. EE then stopped the machine and went to help the member, he then slipped also. Both men went back to work and finished the day. EE missed the next two days and called in stating he had a broken hip.

Employee was getting into pickup truck when his knee ""came out of the socket"".

Struck against a moving object

EE states that he was hurt while driving a water truck. He stated that he hit a bump in the road and the seat bottomed out, causing him lower back pain. On 10/20/08, he was diagnosed with a strained lower back.

Fall down stairs

EE was walking down the stairway, when he slipped on the second step from the top of the lanch of the HWM. As he fell, his leg went under him, causing his right knee to twist. He then contacted his supervisor and was transported to Boone Mem hospital, where he was diagnosed as having torn ligaments in his right knee.

EE was stepping down from the bumper of the service truck and slipped, twisting his knee. EE tried to finish his shift, but couldn't due to the pain. He then went to the boone memorial hospital where he was refered to a specialist. He then met with the specialist on August 28, 2008. He was put off work until Sept. 2, 2008. He was diagnosed as having a bruised bone.

Employee was mounting dozer when he lost his footing, he caught himself by grabbing the handrail and twisting his wrist.

Employee was cutting trees, when EE was walking from one tree to another, he fell on his chainsaw.

Employee was filling up a bulk tank on service truck, when he started to exit the back of the truck he lost his footing and fell striking his rib cage against the top of the ladder. He was transported to hospital and diagnosed with two broken ribs. This accident was discovered in an internal audit as not being reported.

While brazing fitting on 5230 hot hydraulic oil came out of a line and hit left knee causing a 2nd degree burn.

EE was putting anti-freeze into the radiator of a D-11 dozer. He was standing on the dozer cowl platform when he stepped down, slipping on the track pad on the dozer and fell to the ground, causing a laceration to his right elbow, a twisted knee and a sore back.

Employee was standing on rock, loading primers into a boulder. Lost footing and fell on tailbone, hurting back.

EE was adjusting side mirror while holding onto grab handle of truck. Grab handle support failed, allowing EE to fall to the ground.

Employee was opening hood on bulk truck when he felt ""pain in his left side"". Worked throughout the day, but went to hospital the next day as the pain was getting worse.

While using an air gun to install bolts in a 992G, dirt blew into right eye.

Employee stated that he was attempting to stand from a prone position, as he stood and moved to his right side his knee twisted and popped. He stated that he immediately experienced pain and swelling.

Employee stated that while traveling in roadway entering into a right-hand curve, a right rear inside tire blew out. She further stated that the incident caused the truck to shift to the left, causing her to bump her left arm and side of head against the window.

Employee stated that she was driving down a steep grade. The truck began gaining speed. She applied brakes but could not slow truck. She attempted to steer the truck into the escape ramp, but failed to do so turning vehicle onto its side.** No injury Reported**

He was installing a drill tower stand on a Drill Tech Drill the stand slipped & mashed his left hand. Resulting in a cut.

The adapter from the boom slid off bucket and hit into the EE's side. Bruised his ribs.

He was walking the flag line of the cut and fell over a tree and twisted his ankle. The ankle was believed to be broken.

Employee was loading blast holes. He stepped on a slick rock, fell and twisted his ankle. Treated at local E.R. X-rays, given anti-inflammatory meds, light duty.
