EE had a metal strip dogged down and when he turned around the dog broke loose and hit him in the back. He did not report then. Reported 10/16/13.

It appears dozer went over a big rock causing dozer to turn sideways laying it over on its side.
EE was climbing on dozer. He stepped in grease and his foot slipped. He caught himself injuring his shoulder.

Greaser told me he didn't know how or where he hurt his back.
Employee told me at the end of the shift, 1:30 A.M., that his back was hurting. He went on to say that earlier in the shift, the loader operator had jarred his rock truck with a rock while loading the truck. He said he may go to the doctor to get checked out some time the next morning.
Dozer operator was working on fill on downhill slope. The 777 rock truck had turned and was backing down grade when his brakes failed. The truck hit the roll cab of the dozer. The truck was loaded.
EE was operating bulldozer in front of the 1900 excavator trying to dislodge a large rock. The dozer spun around to the left and shot the rock away under him and he came down with the rock landing on the left side of the dozer.

Stepping off of gravel pile - twisted right ankle
While cutting plastic wire tie the knife slipped and cut left finger.
EE went into parts trailer looking for 2 gasket, I went into trailers looking for him and found him lying in the floor. He was not breathing and had no pulse and fluid was coming from his nose and mouth.
EE had removed tire from truck. The tire started to fall and by reflex the EE tried to catch it. The tire was too heavy and it sprained muscles in his back
Mud was on steps of service truck causing EE to slip when getting in truck, hitting his face on the bottom door jam, knocking his front tooth loose.

EE was removing teeth to be replaced on WA600 loader unit 216. After removing the pin he was hitting the tooth with a hammer when a piece of the tooth came off and hit him in the left arm, just above the bend below the bicep, embedded below the skin.
Trying to drive a pin out of a loader tooth, the mechanic was hitting a ball pin hammer with a larger hammer. A piece of metal sheared off of one of the hammers and went into the mechanic's upper arm.
He was removing a ball stud and steering jack, when it came loose suddenly it hit him and sprayed oil and dirt into his eyes.
While operating air impact wrench, the wrench slipped from his grip and cut left hand.
EE was hooking his welder leads up, it grounded out. The brass connector blew up and put 2 pieces of brass into his left eye.
Another EE pinched the EE on his leg, EE jumped and twisted his left knee.

Injured put his hand behind the seat to a place where there was an opening to the air cond. blower in order to cool his hand. There was no barrier and the fan cut his finger.
Started down haul road when rain hit. Truck started sliding and went sideways. Rear tires gripped causing truck to overturn on drivers side. Operator had seat belt on and there was no injury.

EE was walking around his truck, due to a previous accident, his left knee gave out. He fell to the ground, hitting a crib block with his left knee.
The loader dropped rock into bed of truck, shaking the driver and injuring the drivers back.
Working on crusher using a slate bar, prying on roller to put bearing on. Slate bar slipped causing employee to pull left shoulder chest muscle.

EE WAS USING A BOX FOR A STEPLADDER. THE BOX COLLAPSED & SHE FELL TO THE FLOOR. PAIN IN NECK, ARM, & LOWER BACK AREA.
MECHANIC HAD HOOD RAISED ON HITACHI EXCAVATOR LOOKING FOR HYDRAULIC LEAK. OPERATOR MOVED THE MACHINE A LITTLE TO ACTIVATE LEAK. THE HOOD FELL ON MECHANIC'S HAND. MASHED HIS HAND WITH SMALL LACERATIONS.
EMPLOYEE WAS PERFORMING MAINTENANCE ON DOZER. THE WRENCH BROKE THE FALL BACKWARDS INTO A GUARD. BRUISED HIS BACK. DIDN'T NOTIFY ANYONE UNTIL 10-01-01.
EMPLOYEE DROVE DOZER OFF A BENCH ONTO SHOT MATERIAL DROP AT APPROX 15 VERTICAL FEET. PULLED TENDONS IN SHOULDER. WAS RELEASED BY PHYSICIAN TO RETURN TO WORK ON 10-01-01, NEVER RETURNED.
THE LOADER CAUGHT FIRE AND OPERATOR DISMOUNTED. HE JUMPED APPROXIMATELY 5' TO GROUND STRAINING HIS LEG AND BRUISING HIS HEEL.
