The oil for the Girth Gear caught fire. Employees slowed the Kiln down and got the fire out. The Girth Gear is encased in steel. Once the fire was out employees removed the guarding and cleaned it out.

The employee was using a scraper to clean ash from the sidewalls of a damper when EE slipped and crushed EE's finger along the side of the firing hood. The employee suffered a laceration along with a fractured left ring finger tip.

Employee was performing maintenance on the drive system of the cooler discharge scalping screen. The equipment was shutdown and locked out. The employee was removing lime buildup from within a guard with his hand. Lime became airborne and entered his right eye. Physician flushed eye and gave prescription eye drop dosage requirements.

Employees were cleaning out a plugged conveyor and when the employees lifted the cover for the conveyor up off of the floor the employee felt a strain in his back. Notified of restrictions on April 19th, 2012.

Employee was using a two long pry bars to clean out a plugged transfer chute. Employee's left finger was pinched between the 2 pry bars causing a laceration that required 8 stitches.
Employee was raking ash, the ash rake slipped off the large piece of ash and the employee lost his footing. Employee fell to the floor and landed on his left wrist. Left wrist fractured.

Employee was cutting off old blow pipes in the baghouse with a 9 inch grinder. The grinder jumped, hitting the employee in his left knee. Employee had a cut on knee requiring stitches.

Employee was using a fire extinquisher to put out the fire on the C131 belt and had smoke inhalation. The fire was extinquished within the 30 minute timeframe and therefore was not immediately reportable.

Employee was welding inside of a pipe and got a piece of metal in the eye. Doctor removed small piece of metal from employee's eye.

Spiked temperature on kiln trending indicated there was a coal mill puff.
Employee was using ladder to drill holes by dust screw and the ladder tipped causing the employee to fall to the ground. Employee bruised elbow which caused numbness in left hand and fingers. Accident became reportable upon receiving results from electrodiagnostic study showing nerve contusion, employee still performing full job duties. May require surgery.
Fire in the preheater unit. Employee, in a rush to prevent injury from the fire, dropped 6' off ladder to lower platform. Employee suffered the following injuries: lumbar strain, knee/leg strain, cervical strain and contusion of back. MRI disclosed no permanent damage. Employee is expected to make full recovery; return to light duty as of 4/2/07.
Fire in the Lime Kiln Preheater. A contractor was working in the preheater installing refractory. They were using wooden scaffolding and wooden forms which were ignited by an undetermined source. The local fire department responded, but did not enter the preheater, the fire burnt itself out. The fire damaged several steel beams, but no major structural beams were damaged.
Employee was checking the level of the bucket elevator shaft when he slipped on ice accumulation on the grating. As he was falling, his arm caught on the handrail causing a break in the upper right arm.
