CRAWLED INTO LARGE SAND HOPPER TO REMOVE A STONE FROM THE BOTTOM OF THE HOPPER. SAND THAT WAS ON THE SIDES OF THE HOPPER COLLAPSED AS HE WAS IN THE HOPPER COVERING HIM.

EE was working on the suction hose of the water pump. He was checking the bolts that were previously tightened. EE was tightening 6" suction hose on water pump and felt a strain in his neck.

While looking up a rock flew into his left eye. Eyewash was used, then sought medical attention at Marshfield Clinic Wausau Center. Employee was treated for a left corneal abrasion. Given "Tolner" ointment for a 5 dat treatment.
