EE was dumping a load of material out of the box of a haul truck. The cab was turned slightly which caused the cab to tip slowly on the left side causing EE to slam left hip into armrest in cab

Installing a new conveyor belt on a concrete sand stacker. While unrolling the belt that was on a metal shaft, the employee placed the belt on the forks of a forklift. The roll twisted off the forks striking him in the right leg causing contusion and bruising to his right leg inside the knee area.

EE was unloading a PTO shaft for cone @ Metal Crafters for repair. EE of Metal Crafters was helping that person, dropped his end without warning EE. EE was still holding on and caused a shock to hip and lower back. This resulted in a sharp stabbing pain in his lower back and continured discomfort.
