While climbing down ladder of 988 cat loader, employee slipped and grabbed ladder with left arm to hold himself when felt/heard pops.
Employee was using a maul to hit a shank cover on a 988 bucket. A very tiny metal fraqment broke off the maul head and struck employees lower lip apparently embedding in the lower lip.

WHILE DRILLLING A HOLE IN A PIECE OF SHEET METALTHE BIT GOT CAUGHT & THE PIECE METAL SPUN AROUNDON THE BIT. THE SPINNING METAL STRUCK HIS FINGERRESULTING IN A LACERATION ON HIS THUMB
EE WAS CHANGING A WIPER BLADE ON AN ENDLOADER WHEN HE SLIPPED AND FELL TO THE GROUND RESULTING IN A BROKE OR FRACTURED PELVIS.
