THE INDIVIDUAL WAS DRILLING BLASTHOLES. HE TIED DOWN THE HYDRAULIC LEVER SO IT CONTINUED TO OPRATE (ROTATE) BY-BY PASSING A SAFETY DEVICE. HE THEN WENT IN FRONT OF THE DRILL AND STEPPED OVER THE DRILL AUGER. HE BACAME ENTANGLED WITH SOME PART OF HIS CLOTHING HE GOT PULLED INTO THE DRILL AND BECAME SO ENTANGLED THAT HE COULD NOT BREATH. THIS ACCIDENT IS STILL UNDER INVESTIGATION

Initially this was a first aid injury until surgery was required on 3/29/2018. Employee was pushing a camera through pipe. EE was kneeled down pushing hard when ee felt a pain. This incident resulted in surgery to repair hernia.
While grading the road with a loader, employee struck the edge of a concrete pad, causing the loader to stop. Employee hit head against something resulting in a cut on the right side of head requiring stitches.

Liquid spilled off the lid of a 5 gallon bucket of oil while contractor was lifting it out of the containment area. The spilled liquid contacted EE's shirt and pants. As the day progressed EE said it was irritating on the skin.

Associate was cleaning a face when some material fell onto his loader.
Associate was shoveling around skidster that was stuck when he got stuck in the clay around him and cut himself on the skidster. Associate received stitches in the arm.

Associate was cutting belt off fan with knife towards himself and knife slipped cutting hand. Associate was not wearing gloves at the time of the incident. First aid was administered on site and associate went to hospital receiving 6 stitches.

Moving a section of sand chute with a forklift and when one was lifted another chute shifted and fell on ee left foot. He was wearing steel toe boots but is still cut his little toe. Taken to hospital where he received stitches.
Holding bolt from loader with hand, bolt spun cutting left hand middle finger.
EE was hooking up chain to fuel barrel, was getting off of barrel and jammed shoulder
Drill #1 was stuck and EE was using outriggers to lift drill and get rocks under tires. Hand was in open door jam & door to truck slammed on left thumb while raising the outriggers.
EE was changing cutting edges on loader at the shop, tightening bolt with air wrench and finger slipped and got cut by edge of bolt
The associates were moving a retention pond boat by themselves and lost control. The boat slid down a water pipe and struck the associate in the face and arm, resulting in lacerations, requiring stitches.
Operator was opening a rail car door, when wrecking bar slipped out of door clamp and pinched finger on right hand between the bar and the railcar.

EE was removing fuel tank from sweeper in an area with low lighting, and not sure how, but wire cut EE's middle finger on his right hand.
EE while guiding a loader in for cleanup, had his hand on the sump and the loader caught the disconnect stands and pushed it against the sump where his hand was, and his hand was pinched.

Got out of backhoe took 3 steps in loose sand, knee felt different, started to get weak that night and swell. Did not report injury until 7/17/09 to supervisor
While lifting winch cable to unhook from railcar, cable twisted and injured lower back.

Moving manlift in reverse when suddenly engine revved, lift moved in an accelerated speed pinching employee between tire of lift & truck.
Operator was cleaning mine floor with a loader. Plant manager had inspected work area approx 1 1/2 hours prior to incident. Upper half of rib wall fell on front fender (left) damaging it and the access ladder and sandstone came thru windshield. Operator said he did not hit the rib, he saw it falling but could not back out fast enough.

EE was cutting out flexco fasteners on a conveyor belt & was kneeling on the belt. He plugged in the die grinder & didn't look to see that it was switched "ON". The grinder started & cut his knee twice above the right knee. He went to the ER & received stitches & returned to work remaining shift.
EE was replacing take-up pulleys on conveyor in a crouched/bent over position. When he stood up he felt pain in rt knee. He was in position for 1 hr+ & did NOT slip or fall, he twisted knee wrong. He had surgery on rt knee 1/26/05. Lt. duty until 2/7/05.

EE WAS OPERATING A FABCO RENTAL D250E HAUL UNIT THAT DEVELOPED ENGINE FAILURE WHILE TRAVERSING A HILL. IT BEGAN TO ROLL BACKWARDS, PARKING BRAKE WAS ACTIVATED BUT DID NOT HOLD. IT WAS STEERED INTO BANK STOPPING MACHINE ABRUBTLY WHEN IT HIT SAND PILE. HE UNHOOKED SEAT BELT AND CRAWLED OUT OF D250E. PRESHIFT EXAMINATION WAS COMPLETE, ALL SYSTEM CHECKED OUT O.K.
EMPLOYEE UNHOOKED FROM SOUTH SIDE TO MOVE AROUND HOPPER TO NORTH SIDE & STEPPED OFF RAILCAR FALLING TO THE GROUND HEAD FIRST.

EE WAS PRYING ON PIPE TO LIFT PIPE SO LINE COULD BE CLEARED WHEN HE FELT A TWINGE IN HIS BACK.
While dumping sand in mine, ceiling dropped sand onto cab of truck causing cab to compress. Cab roof came in contact with employee.
ON 5-19-01 THE INJURED NOTIFIED HIS SUPERVISOR THAT HE WAS HURT. THE INJURED TOLD THAT HE HURT HIS NECK.HE WAS SENT TO THE DR. IMMEDIATELY. WAS GIVEN MEDICATION AND HAD MINIMAL PHYSICAL THERAP Y.
WHILE DRIVING LOADER ON UNEVEN ROAD, HIT DIP HARD, STRAINED BACK MUSCLE AS A RESULT OF JARRING OPERATOR STATED HE WAS NOT WEARING HIS SAFETY BELT.

HE WAS REMOVING A GUARD FROM A MOTOR COUPLING WHEN IT KICKED OUT AND HIT HIM IN THE ELBOW AND FOREARM.
WAS SHOVELING SAND CLEANING CONVEYORS TAIL PULLEY, TURNED TO THROW SAND AND SOMETHING POPPED IN BACK.
PULLED A MUSCLE WHILE LIFTING A PART WEIGHING APPROX. 50 LBS. CAUSED SEVERE BACK SPASM.
EE WAS WALKING ON SCREENTOWER FLOOR WHICH HAD SAND ON IT. AS HE WALKED BY A WALKWAY STEP HIS LEFT KNEE TWISTED PERHAPS AS HE MOVED TO WALK AROUND WALKWAY. POSSIBLE LIGAMENT TENDON TEARS RESULT ED.
RUSHING TO OPEN A SLIDE VALVE, EE STRUCK THE TOP OF HIS HEAD ON A LOW-HANGING BEAM. THE CONTACT CUT A FLAP OF SCALP, REQUIRING SIX STAPLES.
