Employee was opening a guard to access a tail pulley on a stopped belt. The hinged guard was stuck shut. As he was pulling on it to free guard it sprung open and struck him in the mouth, damaging 4 teeth.

TESTING OF SHAKER SCREEN AFTER REPAIRS, EMPLOYEEPLACED HAND ON BELT DRIVEN PULLEY. PULLEY IS GROUNDED BY LOCATION (OVER 7' FROM GROUND) EQUIP WAS LOCKED OUT, EMPLOYEE WAS CLIMBING DOWN AND CAL LED FOR A START UP CHECK. EE WAS NOT COMPLETELY CLEAR BEFORE START UP.
