Employee was opening a guard to access a tail pulley on a stopped belt. The hinged guard was stuck shut. As he was pulling on it to free guard it sprung open and struck him in the mouth, damaging 4 teeth.

PER THE ATTACHED WORKER'S COMP LETTER, EE WAS RUNNING DOWN AN EMBANKMENT TO SHUT DOWN TWO CONVEYORS WHEN SHE SLIPPED AND FELL TWISTING LEFT KNEE AND LEG (HIP).
EE WAS PULLING ON A TWISTED BOOMER CHAIN. THE CHAIN GAVE WAY AND EE FELL FROM A STANDING POSITION TO THE GROUND THE INITIAL DETERMINATION WAS A SPRAINED ANKLE BY A DR. IT WAS RECOMMENDED THAT HE REST FR A FEW DAYS AND RETURN TO WORK. HE SOUGHT A SECOND OPINION WHO LOOKED AT THE XRAYS AND DETERMINE A STRAINED LIGAMENT, NOW THEY ARE INDICATING A BROKEN ANKLE.

THE INDIVIDUAL WAS CUTTING CONVEYOR BELTING WITH A NEW KNIFE BLADE. KNFIE SLIPPED LOOSE AND CUT TIP OF FINGER. EE WAS TAKEN TO THE LOCAL HOSPITAL. HE RECEIVED FOUR STITCHES, WAS BANDAGED UP AND SEN TBACK TO WORK.
CHAIN SLIPPED OUT OF HAND ANAD BOOMER SWUNG UPA ND HIT HIM IN THE FOREHEAD. EE WAS TREATED AT THE LOCAL HOSPITAL. HE RECEIVED SEVEN STITCHES AND WAS SENT BACK TO WORK. NO CONCUUSSION OR SER IOUS INJURY.
