EMPLOYEE WAS WORKING ON A PORTABLE CRUSHER WITH A PRYER WHEN HE AGGRAVATED AN OLD RT. SHOULDER INJURY FROM FORMER EMPLOYER.
DESCENDING A LADDER AT RADIAL STACKER FOR CRUSHER, SLIPPED ON BOTTOM RUNG & OVER EXTENDED HIS (L) KNEE. HE DID NOT REQUEST MEDICAL TREATMENT UNTIL 3/18/04. CLINIC DETERMINED SPRAIN TO (L) KNEE & HE RETURNED THE SAME DAY TO REGULAR DUTY.

WORKER WAS DRIVING A VOLVO DUMP LOADING UNDER A 345 BACKHOE, A LARGE ROCK BOUNCED OFF THE BED AND STRUCK THE PASSENGER WINDOW CAUSING BROKEN GLASS TO LACERATE HIS RIGHT UPPER ARM.
INJURED WORKER WAS WALKING ON CONVEYOR PLATFORM, FAILED TO FACE THE STEPLADDER WHILE CLIMBING DOWN, LOST FOOTING AND FELL ONTO GROUND CAUSING RT. SHOULDER SPRAIN.
