Stepped on a metal guard, it gave way and he slipped and fell 4 feet to the ground and landed on his back on a plastic pipe.
Removing screens from the shaker. The screen was handed to EE, as EE was backing up hit boot got stuck in the mud causing him to fall backwards with screen landing on top of him tweaking his back.
Turned machine off and tightened belt. Turned machine on to see if it was working and got the tips of his right index & middle finger caught.
Cutting rubber with box cutter. Received 2 stitches.
Flash burn from welding.
Cutting on a piece of steel with proper PPE on. Went to rotate the steel and it cut thru my glove and cut my fingernail.
