Stepped on a metal guard, it gave way and he slipped and fell 4 feet to the ground and landed on his back on a plastic pipe.
Removing screens from the shaker. The screen was handed to EE, as EE was backing up hit boot got stuck in the mud causing him to fall backwards with screen landing on top of him tweaking his back.
Turned machine off and tightened belt. Turned machine on to see if it was working and got the tips of his right index & middle finger caught.

I. NAME WAS PUTTING THE SLUICE BOX BACK TOGETHER AFTER CLEANING. HE LIFTED HIS KNEE AND RESTED IT ON THE SIDE OF THE SLUICE BOX AND LEANED OVER TO PULL THE RIFFLE SECTION UP INTO THE SLUICE BOX. HIS KKEE POPPED OU OR WAS SPRAINED.
