Employee was stripping the outer insulation off of #6 gauge submersable electric wire (outer diameter 1-1/16") utilizing a razor knife. The knife blade slipped/skipped off the poly-vinyl insulation. Upon slipping off of the insulation, the knife blade came in contact with the inner left leg, approx 4" above the knee, causing a laceration.
While walking, the ee slipped and fell on the snow. The ee attempted to arrest his fall with his right hand. He jammed his right index when his hand contacted ground. This is a first aid occurrence.

EE WAS BEGINNIGN TO TROUBLESHOOT A REPORTED STEERING PROBLEM WITH TRUCK, EE WAS SITTING IN SEATM RESTING LEFT ARM ON STEERING WHEEL WHILE REMOVING SHROUD SCREWS UNDER CONSOLE, WHEEL INDEPENDENTLY TURNED THROUGH2-3 REVS, LEFT ARM BECAME CAUGHT IN WHEEL AND WEDGED AGAINST RETARDER LEVER, ARM BROKE BETWEEN ELBOW AND SHOULDER.

WHILE DOING A PRE-TRIP INSPECTION EMPLOYEE STARTED TO CLIMB INTO HIS TRACTOR & HIS FEET SLIPPED. EE FELL & HIT HIS LEFT KNEE ON THE STEP WHILE CATCHING HIMSELF WITH THE HANDRAIL.
OPERATOR WAS CHECKING OIL IN DOZER #3 (D2-3), SLIPPED OFF THE LEFT SIDE OF DOZER, FELL BETWEEN TRACK AND PUSH ARM. INJURED LEFT HIP & PINKY FINGER, HAD ABRASIONS ON RIGHT SHIN.
WHILE WALKING FROM THE TRUCK TO THE SHOT AND HE STEPPED INTO A HOLE THAT RESULTED IN AN INJURY.
