The shift crew was changing sorting screens on a sorting bed. The injured employee was guiding the suspended screen to a storage stack. As ee pulled on the suspended screen groin was caught between the screen and the stack. EE received painful contusions to groin and missed two days of work. EE has returned to full duties.

The injured employee was part of a team replacing a sizing screen on a Symons screening machine. While moving the replacement screen into place the edge of the screen caught on a machine guard. The employee pulled on the guard to release the screen and left hand was pinched between the screen frame and the guard. Employee received a laceration that required stitches.

THE FINGERS OF THE HILIFT WERE ELEVATED ABOUT 4 FEET, AND THE RIGHT FINGER GLANCED HIM AND THREW HI M BACKWARD, AND THEN THE RIGHT FRONT TIRE HIT HIM ON THE LEFT LEG, THEN THE RIGHT REAR TIRE HIT HIS LEFT SHOULDER KNOCKING HIM TO THE GROUND.

WATKINS WAS STANDING ON TOP OF THE ROCKS WHICH WERE STUCK IN THE PRIMARY CRUSHER JAW. HE WAS TAKING A LOOK AS TO HOW TO REMOVE THE ROCKS WHEN THE JAW STARTED TO CRUSH THE ROCKS SO HE STEPPED O FF THE ROCKS AND STOOD ON THE SCALPER SIDE OF THE JAW. THE VIBRATION FROM THE CRUSHING CAUSED A FEW ROCKS TO FALL OFF THE FEEDER. ONE OF THE ROCKS STRUCK HIM ON THE RIGHT SHOULDER CAUSING HIM

CAMACHO WAS HELPING REPAIR PRIMARY CRUSHER MAIN SHAFT BEARING. IN ATTEMPTING TO CROSSOVER FROM THE FLYWHEEL SIDE OF THE CRUSHER TO THE CRUSHER PLATFORM, HE GRABBED A 2 X 4 LUMBER, ONE END OF W HICH WAS RESTING ON A METAL HOLDER WHILE THE OTHER WAS RESTING ON A 2 X 4 FRAME. THE 2 X 4 FELL OFF THE FRAME WHEN HE GRABBED IT WITH HIS RIGHT HAND AND IT WAS PINCHED IN THE METAL HOLDER.
