Over-exertion in lifting objects

Employee was removing clamps and screen from primary crusher shaker when EE felt a pull and pain in the lower abdomen and groin area. EE failed to immediately report the incident and worked the entire shift.

Fall down stairs

Employee climbed a set of stairs that was being fabricated. Stairs was not yet anchored to prevent tipping. Employee was not part of this project. Employee climbed to the top landing of the steps, about 5 feet above the ground. Stairs tipped due to the employee's weight. Employee fell and sprained both ankles.
