While trying to pull out a stone that was stuck in the crusher, the stone came loose and hit him in the left muscle. [Per phone call to reporting party, it was his left leg,contusion.]

The employee said he was feeling pain in the lower back and his left foot about two weeks ago. (Note: This was not an accident)

While repairing a loader he slip and he hurt his left ankle.

While walking around the machine that crushed stone I feel a pain in the bone. I could walk.

Bicep sprain due to carrying box of large screws at work (20 lb box).

Unclogging the fines silo with the ""vara"", some of the fines fell over his face. He put the ""vara"" away and stumbled with his feet falling to the floor over his right side arm.

Vibrator's frame angle fell and hit his left hand big finger, injuring it.

Cleaning inside cone crusher, slipped back and hit it. Felt pain on left side ribs & lungs area.

While working with a heavy bar, the bar slipped and he got a laceration of 5th finger left hand.

Cleaning a cone crusher. He slipped and hit his knee on crusher.

While climbing the stairs he slipped and he hit the front of his head with the handrail.

While loading a truck with rock (boulders) one slip around and trapped the left leg. at the back.

While carrying a box of screws the left foot got through a hole and he suffered a cut. Started rest at home by state insurance policy recommendation on 10 Nov 2005. Worked on until 9 Nov 2005.

AT ENTRANCE TO CLEAN GRAVEL BASE BOX THE EE STEPPED OVER A WOOD PLANK WHICH RIVETED AND HIT HIS RIGHT SHOULDER. THE EE ALLEGED FEELING PAIN IN FRONT AND BACK OF RIGHT SHOULDER & CHEST.

THE SUBJECT WAS PLACING IN POSITION WITH THE BARREL ROCK -DRILL TO DRILL A BOULDER FOR INSTALLATION OF A COIL HOOK. SUDDENLY THE ROCK-DRILL SLIPPED FROM HIS HANDS AND HIT HIS LEFT HAND LITT LE FINGER.

WHILE CHANGING THE FURUKAWA BITS, THE BITS SLIP AND HIT THE LEFT HAND FINGERS.

WHILE CHANGING THE WELDING MACHINE ELECTROD, HE TOOK THE MACHINE CABLE AND IT FELL OVER A METAL PLATE. WHEN TAKING BACK THE CABLE, HE GOT BURNS ON THE RIGHT HAND AND FINGERS.

A CRANE LIFTING A BEAM, HE HELD ONE SIDE TO GUIDE IT, THEN HE WENT OFF BALANCE BACKWARD & WITH HIS LEFT FOOT HE AFFIRMED ON GROUND FULL OF STONES & BENT HIS LEFT FOOT AT ANKLE LEVEL.

USING DEGREASER TO WASH HIS EQUIPMENT, THE DEGREASER SOFTENED THE PAINT OVER THE MACHINE TOP DECK, WHEN HE STEPPED FORWARD THE PAINT WAS LOOSENED BY THE DEGREASER AND HE SLIPPED OFF BALANCE FALLING OVER THE DIESEL TANK AND BATTERY AREA OF THE MACHINE JUST BELOW AND BY THE WALK DECK.

REPORTED ON JUNE 21-01 THAT HE CAN NO LONGER DRIVE WITH THE PAIN IN HIS NECK AND UPPER SHOULDER WHEN DOING SO. HE HAS BE UNDER PRIVATE MEDICAL TREATMENT SINCE MARCH 2001.

WHILE CLEANING THE CONVEYOR A STONE HIT HIM IN THE JAW.

WHILE WORKING WITH A SLING THE SLING SLIP & HIT THE EE ON THE NOSE

WHILE STEPPING INTO A PICKUP, HE SLIPPED AND HIT AN IRON BAR AND HE LOST 1 TOOTH AND 2 MOLERS LOOSENED.

WHILE DRIVING THE TRUCK I HURT MY BACK.

WHILE OILING THE PLANT HE HURT THE THIRD FINGER OF THE LEFT HAND
