On Monday, May 18, in the first shift, the operator of the r-line 1 area, suffered a sprained left shoulder, while placing 50-pound bags of product on a pallet.
In production area a miner was affected when in the task of filling big bags with final product where the thumb of the right hand was pricked between the ear of the big bag and fork section of the forklift

On the 23 of august 2025, at approximately 10:39am, a surface miner sustained a head injury after being accidentally struck by a sledgehammer that was in use during repair of entrance gate. The miner was moving a ladder when the sledgehammer fell from the top of the ladder, directly striking the top of their head. They suffered a scalp laceration with bleeding.

An independent contractor was struck, causing an injury to their right leg at the ankle, by a section of the new dust collector duct, while unloading duct sections from the truck bed.
Miner walked over a pallet straining left ankle.
Operator of the machine that wrap pallets with product, was cleaning the machine in the transfer part, using a paint brush EE puts EE's right arm through a space between the roller and the chain that moves the pallet in to position, inadvertently touches a censor which activates the chain, and it goes up puncturing EE's arm, EE manages to get out by touching the censor again.
The task is performed by the back of the machine climbing on a step to reach and put the bags. Apparently, EE stepped badly on the step, which caused the step to turn causing the fall on the guard of the strap, hitting the right forearm, causing severe pain. Affected area was put on ice and sent to be seen by a doctor.
Walking thru mine outside facilities miner slipped at a zone that has sediments and was wet. EE made a jerky movement that made EE bend EE's left knee falling on that side to the ground. EE finished EE's shift that day, but next day EE went seeking medical attention. Doctor gave a medical leave until 02/13/2023. EE's left knee was swollen.

a miner stepping down from the loader, in the last step of the ladder twisted an ankle (right foot) causing them to fall backwards (approx. 3 feet high) to the ground, the miner walked to the EHS office to informed what happened and informed that they have pain in the lower back, miner was referred to HR for documentation and sent to hospital for evaluation
Using their strength, EE tried to pull a heavy steel plate that was on the floor, to close a gap. EE strained right arm elbow, a bump was visually observed. Arm was immobilized and miner sought medical attention.
Welder was standing on an area outside, on the facilities, waiting turn to continue the task when the wind blew dust particle from other area to EE's face affecting both eyes (EE said that EE was wearing safety glasses) EE went to wash but the burning sensation still persisted and left work to look for medical assistance
Miner was removing a cap screw by holding a metal screw in right hand and a drilling hammer in left hand, when miner try to hit the screw instead miner mash index finger of the right hand, at the time miner said miner is ok and complete shift, next day was absent and call to report that miner will seek medical attention. Doctor only indicated that miner has a bruise

A crane overturned in the sand plant area trying to overturn an industrial dust collection, the miner hit hard inside the cabin of the crane, losing consciousness for a few minutes. The miner was treated by paramedics and taken to the hospital, hours later miner was discharged because miner did not show lesions on the X-rays.

The welder was working with the manual grinder and even with the safety glasses a metal particle fell into the right eye.
The employee went down the stairs with a hammer in hand. At the end of the ladder the employee jumped and the finger of employee's right hand was caught between the hammer and the ladder. This caused a fracture of the second finger of the right hand.
In the bagging operation a sack broke. The material with cement falls on the face of the operator and the eye was inflamed by apparently allergic reaction. The operator was using EE's safety equipment.
Right Hand punctured with a chain conveyor. The operator wanted to remove a broken bag from the conveyor.
When the warehouse attendant was manually placing glass blocks boxes on a wood pallet, other boxes fell and hit ee in the legs.

In the cleaning of the working area, The employee touches the hand hoe with foot and the stick hits ee in the face. The impact caused a wound that required three stitches in the eyebrow of the right eye.

In the process of filling 50 lbs bags, the nozzle of the baler get blocked. The operator inserts a metal rod to remove the blockage. It is when the rod hits the right hand of the operator.
The employee was using a tool to exert force to loosen a nut from the air compresor, slip and hurt their back.

In the bags packaging equipment, we handle three different bag sizes. When the operator was changing the bag positioning fixture, right hand got pinched in the bag positioning fixture.

The employee was fixing a wrapping machine which had entangled plastic on the film tower. The machine was in Log/Out and the tower break was unrealeased so the employee could move it by hand. He was taking out the plastic from the tower, when it slipped and fell down on a height of 30 inches.

June 2, 2008 the employee was working at Q-Line 1 when the machine got stuck. He proceed opening the main door of the machine to check the purpose of the obstruction from the gate of the mixer compartment. He tried to fix the gate and the mixer got activated hitting the right hand on the wrist.
The EE started his regular shift. He got one of his fingers on his right hand stuck while unloading the bulk bags.
The employee walked thru the warehouse to the bathroom. She can't identify that in front of her we have a stairway. She hit the stairway with her forehead.

THE EMPLOYEE WAS MOVING BAGS OF CEMENT FROM ONE PLACE TO ANOTHER. DURING THAT PROCESS HE FELT A SEVERE BACKACHE.
THE EMPLOYEE ALLEGED THAT HE WAS UNDER THE VERTICAL SCREW CONVEYOR IN THE REPAIR LINE. HE WAS MIXING CEMENT WHEN HE FELT THE CEMENT FROM THE TOP DROP AROUND HIS BODY. HE ALLEGED THAT ONE OF THE PIPES ABOVE HIM WAS DAMAGED.

THE EE ALLEGES THAT WHILE HE WAS PREPARING A MIX OF GROUT, WHEN HE MOVED THE BAG OF CEMENT OF APPROX. 30#, HE FELT A STRONG PAIN IN THE LEFT SIDE OF HIS RIB CAGE. HE IS IN MUC PAINA ND CA HARD LY MOVE.
THE EE WAS MOVING SOME BAGS FROM THE DUST COLLECTOR AND HE TRIED TO REMOVE THE SCREENS THAT PROTECT THE BAGS AND HE HURT HIS WAIS. HE FEELS MUCH PAIN AND CAN HARDLY WALK.
THE EE WAS CHECKING A MOTOR THAT WAS VIBRATING AND HE WAS REINSTALLING THE COVER OF THE MOTOR, THE COVER SLIPPED, AND THE EE TRIED TO HOLD IT AND HURT HIS BACK.
THE EMPLOYEE WAS PACKING THE CEMENT PRODUCTS IN A BUG BAG, ANOTHER EE WAS MOVING THE PALLET WITH THE FORKLIFT. NO ONE REALIZED THAT HIS FOOT WAS UNDER THE FORKLIFT AND HE LOWERED THE FORKLIFT, IT PINCHED HIS RIGHT THUMB.
THE EE WAS LIFTING 50 LBS. BAGS AND EMPTYING ITS CONTENTS INTO A MIXER WHEN HE FELT DISCOMFORT IN HIS RIGHT TESTICLE. THE PAIN HAS CONTINUED, THEREFORE MAKING IT IMPOSSIBLE FOR HIM TO CARRY WE IGHT.
THE EE ALLEDGES THAT SHE FEELS A CONSTANT PAIN ON HER RIGHT ARM FROM HAND TO SHOULDER. SHE IS NOT ABLE TO MAKE BRISK MOVEMENTS AND HAS LOST STRENGTH.
THE EMPLOYEE ALLEGES THAT WHILE HE WAS GOING UP THE STAIRS, HE FELT A STRONG PAIN IN HIS LEFT KNEE. HE DID NOT HURT IT, BUT HE NEEDS TO USE THE STAIRS ON A DAILY BASIS WHICH WILL AFFECT HIS KN EE.
THE INJURED EMPLOYEE ALLEDGES THAT WHILE HE WAS REPAIRING THE MOTOR OF THE PRIMARY CRUSHER & REMOVING THE SHAFT WITH AN IRON BAR, HE SQUEEZED & CRUSHED HIS RIGHT PINKY FINGER WITH THE BAR AND METAL GRILL WORK.
THE EE ALLEDGES THAT WHILE HE WAS WALKING ON THE ROOF OF THE SILO, HE SLIPPED AND FELL BACKWARDS, HURTING HIS BACK.
THIS IS NOT AN OCCUPATIONAL ACCIDENT REPORT. NEVERTHELESS, THE EMPLOYEE ALLEGES THAT SHE FEELS MUCH PAIN WHEN SHE PICKS UP EMPTY SACKS TO HAND OUT TO HER FELLOW WORKERS. SHE ALSO FEELS A LOT OF PAIN ON HER LOWER BACK AND IN HER RIGHT LEG.
THE EMPLOYEE WAS MOVING HEAVY SACKS (94 LBS) WITH ANOTHER CO-WORKER AND HE FELT A MUSCULAR TEARING IN HIS LEFT WRIST. HE IS IN A LOT OF PAIN AND CAN'T UTILIZE HIS HAND.

WHILE EMPLOYEE WAS MIXING HEAVY MATERIALS, HE EXPERIENCED A STRONG PAIN IN HIS RIGHT TESTICLE WHEN HE LIFTED THE HEAVY WEIGHT. THE TESTICLE REMAINS SWOLLEN & IT FEELS VERY UNCOMFORTABLE.
THE EMPLOYEE ALLEGES THAT WHILE HE WAS CARRYING BOXES OF CHANNEL SCREED, HE FELT A STRONG PAIN IN BACK.
THE EE ALLEGES THAT WHILE HE WAS HEATING A PLASTIC COVER, HE DID NOT REALIZE THAT THERE WAS A BROKEN PALLET WITH NAILS STICKING OUT. HE STEPPED ON A NAIL WITH HIS LEFT FOOT.
THE EE WAS SCREWING A GREASE HOSE WHEN HE TURNED AND HIT HIS LEFT ELBOW WITH THE EDGE OF THE CONVEYOR.
EMPLOYEE ALLEGES THAT WHILE HE WA OPERATING THE SAND DRYER, HE FELT A CONGESTION ALL OVER HIS FACE AND IMMEDIATELY THE LEFT SIDE OF HIS FACE BEGAN TO SWELL. THE SWELLING BEGAN TO RECEDE, BUT H E FELT A SHARP PAIN IN HIS LEFT LEG WHICH CAUSEDTHAT HE CANNOT WALK PROPERLY.
THE EE WAS MOVING SOME PALLETS WITH A BAR. HE MADE AN EXCESS FORCE AND FELT AN INTENSIVE PAIN IN THE LOWER PART OF HIS WAIST AND HIS LEFT LEG.
THE EMPLOYEE WAS MOVING SOME GLUE SACKS (50 POUNDS EACH) AND SUDDENLY HE FELT A STRONG PAIN IN THE LOWER BACK.
THE EE WAS PACKING SACKS ONTO A PALLETS WHEN SUDDENLY HE FELT A SHARP PAIN, AS IF HIS MUSCLE WAS TEARING. THE PAIN STARTED IN HIS CHEST LEFT SIDE AND WENT ALL THE WAY DOWN TO THE LEFT HAND. HE CONTINUED TO WORK BUT THE PAIN INCREASED. HE ISIN A PERIOD OF REST.
THE EE WAS CLOSING A VALVE OF THE PRODUCTION MACHINERY, THEY WERE USING A PARING CHISEL. THIS INSTRUMENT GOT LOOSE AND HIT HIM IN THE UPPER PART OF HIS NOSE.
EMPLOYEE WAS CHECKING A COMPRESSOR, WHEN HE WAS CLOSING A VALVE, HE HURT HIS WAIST AND THIS PRODUCED SHARP PAIN IN THIS AREA. HE CONTINUED TO WORK BUT THE PAIN INCREASED.

THE EE ALLEGES TODAY JULY 27 1999 THAT ON JULY 21, HE FELT A STRONG BACK-ACHE. HE DID NOT REPORT IT TO HIS SUPERVISOR, AND NO OCCUPATIONAL INCIDENT EXISTS. WE HAVE KNOWLEDGE THAT JULY 8, HE FE LL IN HIS HOMES BATHROOM, BASED UPON WHAT HE REPORTED TO USE.
EMPLOYEE ALLEGES THAT FOR SOME TIME HE HAS BEEN FEELING DISCOMFORT IN BOTH HANDS AND OCCASSIONALLY DOES NOT HAVE THE CAPACITY TO RAISE OR MOVE HEAVY OBJECTS. HIS HANDS ARE CRAMPED AND CANNOT C LOSE.
THE EE WAS WALKING IN THE PLANT. SOME EE'S WERECLEANING THE AREA & HAD REMOVED THE FLOOR GRATINGS. EE DID NOT NOTICE & FELL ISIDE THE HOPPER INJURING HER RIGHT LEG
THE EE WAS UNCOVERING THE MILL WITH A ROD, SUDDENLY THE ROD TURNED OVER HIM CAUSING A LACERATION INSIDE HIS RIGHT EYE. CAUSING ALSO A STRONG PAIN IN THE EYELID AREA, AND GET WORST AS HE MOVES IT.
EE WAS ORGAINIZING SOME BOXES OF GLASS BLOCKS. HE WAS PUTTING THEM ONE ON TOP OF THE OTHER. SUDDENLY, HE PULLED OUT HIS RT HAND AND HE INJURED IT WITH ONE BLOCK THAT WAS BROKEN.

EE INDICATES THAT WHILE HE WAS MOVING AROUND SOME READY MIX CONCRETE BAGS TO STACK THEM ON A PALLETE, HE FELT A SHARP PAIN ON HIS BACK(WAIST). AFTER THIS THE CONVEYOR STOPPED BECAUSE AN EXCESS OF BAGS ON THE CONVEYOR BELT, AND WHEN IT RESTARTED THEY (SACKS), FELL ON HIM AND HE FELT THE PAIN MORE SHARPLY.

EE WAS POLISHING A TANK WHEN THE POLISHER DISC JUMPED OFF CAUSING AN INJURY TO HIS LEFT ARM.
