54736YEE WAS FEEDING COAL CRUSHER HOPPER FOR SOME UNKNOWN REASON HE WENT TO THE CLINKER STORAGE AREA USING AN/INCLINED ROADWAY. THE LOADER ROLL OVER AND THE VICTIM'S HEAD WAS CRUSHER WITH THE 58497MENT.
WITNESSES ALLEGE THAT THE EMPLOYEE WAS PREPARING AN AREA FOR THE TEMPORARY STORAGE OF STEEL BEAMS WHICH WERE BEING OFF-LOADED FROM A FLATBED TRUCK. WHILE THOSE BEAMS WERE SUSPENDED OVERHEAD BY NYLON SLINGS, THEY TILTED, SLID THROUGH THE SLINGS AND FELL, IMPACTING THE EMPLOYEE IN THE HEAD (FACE AREA). THE EMPLOYEE WAS TRANSFERRED BY AMBULANCE TO A NEARBY HOSPITAL WHERE HE WAS PRONOU

Se en contraba empleado en el cuarto hidraulico del enfriador poso su mano izquierda entre la Puerta y el marco de entrada. El viento la cerro y impacto sus dedos.
El empleado hacia labores de limpieza a traves de un escotilla de la tolva con espacio limitado el material bajo de forma inadvertida y golpeó la herramienta que a su vez golpió al empleado. EE was cleaning a hopper through a hatch with limited space. Material shifted and hit the tool which in turn hit the EE.
In rigging process of mechanical piece when the crane operator picked up the piece it moved to the front and hit a wood pallet and the wood pallet hit the left foot of the miner.

Employee was was doing mechanical work, with a transport chain on links with chisel and hammer hand tools when a particle of metal was released that flew and embedded in the lower lip. Medical assistance to the employee was made.

Employee was walking near urea dike and tripped on the ground. The employee say no to receive medical attention. Few days after, EE decided go to to receive medical attention.
The employee slipped on wet road.
Employee walked downs stairs slipped on step. No fall. EE has three points of support.
During lubrication process the miner made contact with hot grease at mills number 3.
Employee walked down stairs, at the end of the stairs they slipped and fell to the ground.
Employee who slip with piece of rock and went to ground.
Maintenance employees were working with a large piece of metal in the mechanics shop. The piece moved of the work table, the employee realized that the piece would fall where ee was. The employee evades the piece causing the helmet to fall to the floor and hit the head with the edge of the table. Also, the shin of the left leg is lacerated with the piece that was on the floor.
This report is prepared to comply the requirements of MHSA. 2/22/18, employee alleges that when ee performed the cleaning activities using the skip loader in the transporte#3, hurts ee back after hitting a bean when leaving the area. 2/26/18 employee was referred to Argos Medical provider which referred to workers compensation.
On 1/26/18, employee informs of a severe chest pain. We proceed to transport the employee for medical treatment. On 1/29/18, the employee proceeds to visit EE's personal MD, who recommended work rest. On 2/1/18, employee go to the Emerg room do to continue chest pain, where EE was evaluated and diagnostic muscular sprain work related and was referred to workers compensation.

During the process of completing the unblocking of the 4B pre-heating cycle, the production supervisor stepped on the remaining material, trusting that it was cold after applying air to it, resulting in a warming of the boot of the left foot causing a blister.
The employee carrying out the replacement and mounting of the bearing at the central screw conveyor of the Bag house, climbed to a platform with approx. height 3'7". When employee tries to get off it, slips and falls to the ground hitting back and shoulder blade on the right side.
Employee was walking in front Technical offices when employee slip & fall on the road. The road had the present of some liquid in a small perimeter

In the packaging area one operator got faint due to previous medical conditions, when operator fell injured one finger of right hand, needed stitches in finger. Operator has previous medical conditions which are under medical investigation for the faint.

Employee was closing the clinker feeder belt guillotine. When the employee had finished; Employee began walking on the grating walkway to exit the area when employee struck knee against the conveyor belt. The employee got a superficial wound on knee that needed 2 stiches as treatment.
Two employees were making a lifting for engaging an electrical engine; they lifted using the ball Eye ring and the reducer part of the engine. One employee put hand under the suspended load (engine) to install the screw at its base when The Eye ball ring came off the engine making the engine to get down and catching right hand fingers ( middle and ring fingers )
The loader operator introduced left hand in the air conditioning unit. EE suffered a cut on the index finger.

The employee did not take precautions when he closed the door of the elevator and this cause pricked one finger.
The supervisor went to see the pump area were one diesel pump was having problems. At the moment he was getting near to the pump, the valve at the exit of the pump got loose from the fitting and started spray diesel all over the supervisor.
During the third shift the lab technician informs to the plant foreman that she was not feeling well, due a strong smell in the laboratory caused by release of air and oil from the x rays machine compressor in the Lab. The plant foreman reaches Laboratory and felt the strong odor, employee was retired from the area, and compressor was shut off
The employee was walking by "BELT MARGINAL" towards the discharge. Slightly touched his helmet to a pipe passing through the area to 62 inches high, causing the employee to strain his neck. There was a preexisting condition.
The employee was working on the stretch wrapper machine. He had to bring down the tower, do the Lock out and go into the area where the dispenser is. At that point he hit the top sheet dispenser structure with his body and lost his balance getting injured on the left side his eye by hitting the base of the photocell.
Employee alleged discomfort in both knees and hips due to the frequency of going up and down the cement silos stairs. Employee condition is being medically evaluated to determine if it is related to work.

Employee was working above the old Finish Mill#1 roof in a manlift trying to remove a purlin, He step out of the man lift cage & walked on the roof, the roof collapsed & fell aprx.4 feet hanging from his arms between a structure channel and a cross bar;& rescued by another manlift;he suffers scratches under his arms & hands; he was wearing the PPE including harness fall protection.

Employee was moving a fan frames from shell, supporting other employees in the task when he felt some back discomfort.
Employee was assigned to install the grilled plates on the IKN of the cooler; in the process of installing the plates he felt a sharp pain on his lower back when he extended his body forward during the process. The employee was wearing his PPE.
Employee was cutting with an oxygen/acetylene torch a flat metal sheet in the workshop, while doing the final cut a piece of 14" X 27" X3/8" slide from the workbench falling into his left foot. Employee was wearing his PPE.

Employee was walking down the ramp passing underneath the marginal belt conveyor and slipped loosing his step. He tried to maintain his balance to avoid the fall, he twist his body with force and felt a sudden pain in his lower back. The employee reported immediately to his supervisor and notify discomfort he was feeling in his lower back.

The shift supervisor went to the iron feeder and noticed that it was clogged and raw material mixed with cement. Then the shift supervisor and the shift mechanic started to hit the material with a spear to break material formations and began to go down. They used a hammer and the spear together. After these work the supervisor began to feel discomfort in his left shoulder.
Employee was doing overhead welding under the vacuum truck tank. Suddenly he feel pain in his right shoulder when finishing the job.
Employee was sent to unclog the iron hopper. To loosen the material, the employee hit several times the hopper close to the discharge. When he was doing this operation, failed with the hammer and hit one of the ribs of the hopper with the left thumb causing a cut that was required 4 stitches.
Employee was pulling cable through conduit during a system repair and the bending, pulling, and stretching caused a lower back strain.

Employee was trying to break the plate lock to gain access to their tools because he forgot the keys. During the process, he was using a homemade pipe lance as a bar to pull the plate and break it. He made a strong force and the lance slipped between the door and plate causing the fifth finger to get over the plate edge causing a cut over the nail and finger's body.
Employee was sent to the cooler electrical room for cleaning and maintenance purpose. When finished the job he passes in front of the cooler hydraulic room that had tree leaves and mud in the floor he slipped but not fell and the force made to avoid the fall cause the right leg muscle to strain.
A supervisor was working with the cooler that stop suddenly. To emptied the cooler two hydraulic jacks of approximately 10-20 lbs. were used in the drive #2 area. The two jacks barely move the grates and supervisor decides to use a big hydraulic jack, the jack slips forward and the supervisor tried to pull it back and feel a sharp pain in his back and right leg.
Employee was working with the Roller Mill pneumatic crane chains. He tried to drive a stretch of chains up by hands to unravel the base of the crane, and during the action he injured his back. He was complaining of back pain.
Employee was walking in direction to the crossbelt analyzer and passed close to the kiln inlet seal located on the ground floor and step on a small pile of limestone material spilled (aprox 14"). The pile of material was hot inside and the material enter the boots causing 2nd degree burns to the back of the ankles.
Employee was absent of work due to pain in the left shoulder. He was referred to state workers compensation insurance company by his doctor. He was sent to the company physician for evaluation and they determine to send him to state workers compensation insurance too because they determined that the pain is related to work activities.

Office employee was walking from the parking area to the offices administration building, she slip and fall (same level) in the sidewalk at the entrances of the administration building during heavy rains. Employee was returning from her lunch break. As result of the fall the employee suffers a contusion on her left hand and minor scratches on both knees.

Employee was performing lubrication schedule at the D21 conveyor belt, he reported that while filling the gearbox standing over the conveyor frame he partially lost his balance and to avoid the fall, grab the frame with his left hand. Afterward employee reported pain on his left shoulder.
Employee was changing the conveyor belt #2 of the cement Finish Mill #2; while in the process, employee picked up an end of the chain and launched towards the other side of the belt experiencing an intense pain on his left arm shoulder.
Employee was adjusting belt cleaner of conveyor belt #8 head pulley with an adjustable wrench hand tool; the wrench slipped from his hand impacting the base support ribs of the conveyor with his right hand 4th finger tip.
Supervisor was inspecting the pinions of a blower at the mechanical shop. During the inspection he moved the gears by hand and his left hand second finger got pinched between the blower gears. Left hand 2nd finger tip fracture.
Employee was performing belt mounting at the high speed crusher roller. Lockout was in place. Employees were moving the belt roller by hand to get in place the belts. Employee was standing on the pulley base when suddenly slipped and grasp the inside belt with right hand and got trapped between belt & pulley. Employee suffer a finger tip fracture of right hand 4th finger.

Employee was performing a cleaning process to the high speed crushing mass with a hand tool (twibil) when his left hand was smashed between handtool handle and a door frame.
Employee was doing routine lubrication of Roller Mill tires, inside the equipment; his right foot got stuck between the side ring protection bars, lost his balance and fell, bumping himself against the equipment.
Employee was performing an installation of the inspection registry in the discharge hopper of belt 3 in the FM#2. During installation he was looking for support when he stepped back and his left foot fell down thru the vertical ladder opening.
Employee was on his way to the clinker ranch to collect lab sample; inside the ranch after collecting the samples he triped over clinker gravel, lost his balance and felt to the ground on same level. As result of the fall the employee landed over his left arm and suffers a hairline wrist fracture.
EE was feeding the clinker marginal hopper during the 3rd shift. He stepped down from the loader to check on the clinker marginal conveyor belt. There is a small ramp next to the hopper that leads to the clinker transport belt area, on his way down the ramp he slipped and fell, injuring his left knee.
Employee slip and fall over gravel after heavy rains on his way to collect laboratory samples, at the clinker cooler west side ramp.
Supervisor was initiating the start-up process of the Roller Mill,by overload of material in the mill table,he proceeded to rotate the table with the auxiliary motor. The auxiliary motor has a manual coupling to start-up the engine in local; the handle had a pneumatic coupling mechanism; it failed to connect properly causing the handle to bounce back hitting his right hand.
Employee operating the automatic bag filling machine and noticed malfunction of the bag applicator; he removed the guard that protects the pulley and introduced his hand to check the belts; his arm obstructed the photocell device activating the spout mechanism. His left hand got caught between the belt & pulley.
Maintenance work was being performed inside the first chamber of the cement mill #3, several plate liners were being replaced. Employee was welding inside the mill standing on the first rung (bottom) of the access ladder while welding the plates; he stepped down, got tangled on his own boot shoelace & fell backwards, falling over the bed of balls. Employee fell from 12" high.

Employee was preparing area and materials to start working on the big bags packing site. He proceeds to receive a pallet of empty bags, once on location he proceeds to remove the wrapping ropes around the pallet. One of the strings got jammed so he pulled it out with force, the string got loose, the employee lost his balance and felt backward landing on his back.
Sales representative from outside supply company was at the plant taking measurements and quoting for replacements of fire rated doors at the electrical rooms. Walking down the access ramp to the clinker transport electrical room, he slip and fall over some clinker and gravel, and twist his left ankle.
Employee was using a steel wheelbarrow to remove spilled material around conveyor belt #6. The repetitive motions to overturn and empty the wheelbarrow strained his right hand and wrist.

Employee was working in the removal of gear box from the spare drive shaft of E01 chain. Two hydraulic jacks with supplemental pieces were used. While the jacks were in pressure the gear box was heated to clear the interference between the couplings. As the coupling came loose one of the supplements slipped out and hit the employee on his right leg.
Employee was doing routine work on the oil change service ramp. After cleaning the area, he proceeded to service one of the company vehicles, during which he slipped and fell through the service ramp opening (4 ft. high), to the ground and injured himself above the right eyebrow.
Employee was at the electrical room of Mill 3 to reset apron feeder of slag conveyor motor. The breaker lever was tripped and he couldn't reset the lever to the OFF position so he proceeded to open the panel door. He used a screwdriver to push the lever into the OFF position for resetting, then pushed the lever to the ON position when a sudden arc and explosion occurred.

Workers were preparing to remove the roller bearing from the intermediate symetro gear using a tri-jaw hydraulic gear puller. After securing the puller and applying the pressure, they started warming up the roller so it would begin loosening the bearing. On a sudden release of the bearing, one of the puller jaws slung out impacting the worker's left hand and chest.
Employee and supervisor were moving a portable fan which was cooling #9 kiln bearing. In lifting the fan the casing was hot because of radiation from the kiln shell; the employee was not wearing gloves, so he dropped the fan at the contact with the hot metal, trapping his left hand index finger between the edge of the fan and a bin. Causing fracture of the finger.
Employee slip and fell in the parking lot. Walkway was wet and slippery due to water leakage of an underground water pipe. Employee fell hitting his right elbow against the pavement.
Employee was assembling an electrical control panel at the Electrical workshop workbench. To test the control circuit he energized the 120V power source. While fastening the cables with tie straps, his left hand slipped and came in contact with the thyristor cooling fan (3X3 inches), lacerating the tip of his left hand 5th finger.
Employee was opening the clinker feed elevator for bucket repairs; they were adjusting the counterweight guide and the guide beam shifted out of place, hitting his right foot.
Employee was removing a tide rod pin from the roller mill hydraulic piston. While holding the nut wrench in position, his coworker struck the wrench with a 20 pound sledge hammer to loosen the nut. With the impact the wrench slipped out, and pinned his left hand third finger against a steel bin.
Employee was moving a wood pallet and cut his left hand small finger with a scrap of metal. Laceration required stitches.
Employee was placing the babbit (bearing) of Finish Mill #2 electrical motor into the sleeve support. The babbit slipped and pinched his left index finger against the support, causing a puncture wound to his finger.

Production supervisor was preparing to lockout cement mill #2. In the process of performing the lockout, the supervisor had to lower the mill main breaker,the breaker would not lower easily, so he opened the side panel to release the breaker latch,while attempting this action an arc was created and escaped through the open panel, burning his right arm.
Shift supervisor stepped over a water pipe line to check level of a 30 gal vertical rervoir tank; he slipped and fell dislocating his left hand wrist.
Employee referred low back pain probably caused by a slip & fall that he experienced about two months ago, when he tripped over an iron rod while walking in the plant. Medical evaluation indicates lumbar sprain.
Independent truck driver assistant fell behind the cabin of the truck during initial movement of the truck while removing the side straps of the tarp and was roll over by the first set of wheels on the back of the truck.
He stepped over some accumulated material to inspect the space between the loader ramp & the feeder; he stepped back on to the platform and accidentally moved a piece of scrap metal that was lying against a beam; he bent down to check chain conveyor & the piece of metal slipped & fell on his back.
Employee was walking with some difficulty due to gout pain of right knee. On way to get toolbox, EE slips and fell, and twisted knee. EE continues working regular shifts; on 1/31/05, was sent to get medical attention. EE knee condition was aggravated by the excess tension of the knee when EE tries to avoid the fall.
Employee was making perforations to a 1" rubber conveyor belt with the arbor press at the machine shop. After completing various punch, the 3/4" hollow punch bit slip-out from the press hitting the employee right hand third finger.

Employee was helping in moving a 55 gallon oil drum from a wood pallet located at the warehouse entrance; next to the pallet was a garbage metal container. When the employee rotated the drum it turned over and caught his right-hand thumb against the metal container, crushing his thumb.
Plant Foreman was doing his routine shift walk around. On his way down thru the rear stairs from the control room to the clinker storage silos, one of the stair treads broke loose causing the employee to lose his balance; his right leg went thru the opening and he felt frontward, hitting his right knee on the next tread. Employee suffered a muscle sprain and mild knee contusion.
During the plant shutdown work ee was assigned to remove spilled material around the plant, particularly inside clinker cooler, kiln and roller mill. During the clean up work additional shoveling was required to remove compacted clinker from the cooler and compacted raw material from discharge hoppers
