An independent driver drove over the edge of the road in their truck, while exiting the facilities.
Opening a lid from the plant VSI, the employee let it go and it fell on left hand.

On October 6, 2021 around 11:00 am, the miner started to feel ill, we took EE to the ER and it was later determined that the employee suffered a stroke.

Off road truck operator lost control of equipment, turning over its right side.
The miner was tightening track bolts for a bulldozer Komatsu 275AX with a handle and it broke. He fell twisting his ankle.
EE was moving the breaker grid from the impact crusher. The grid struck his hand (left) cutting it.
HE WAS REMOVING FROM THE IMPACT CRUSHER, ONE HAMMER WEDGE. ANOTHER EMPLOYEE USING A STEEL BAR STRUCK ONE FINGER ON HIS RIGHT HAND.
