Subject employee suffered a minor cut on his right hand index finger while mounting a welding trailer on to skid loader hitch to be move to other site for a maintenance routine.

The employee was working changing a screen from the horizontal shaker when he unwillingly moved back and step into the stair way, this caused him to fall and suffer minor abrasions on his arms and hands.

At lunch time when he was sitting on a chair: He reclined back and fell on the floor: breaking his right arm. Since that moment he was has been out of work.

At the beginning of the day he was refueling his excavator when he slipped off the excavator and hit his elbow on the ground.

He was fixing a conveyor belt of the sand plant when he slipped on the belt and his hand got cut.
