Employee was using a grinder and cutting disc to cut a u bolt on a trailer. While cutting the bolt the disc broke striking employee in the wrist and chest area causing lacerations one to the wrist area and one to chest area.

EE was changing belts on dredge when the wrench slipped and EE cut EE's left arm on hose clamp
Employee was cutting an airline hose for truck #1221 with a knife and the knife went through and cut right thumb

EE was taking lug nuts off a truck and the wrench broke, causing ee to fall on right knee onto concrete.

EE was hooking a chain from the loader to the dredge. EE told the Loader Operator when EE was done and to go ahead and backup. When the Loader Operator backed up the Loader Operator backed up a hill which made the bucket come down and EE did not move out of the way and the bucket came down on EE's left foot.
Employee did not report incident to the supervisor until 7/13/17. Employee was servicing a truck when EE's arm brushed a portion underneath the truck knocking dirt loose. Some got into EE's right eye.
Employee was using a grinder in the shop with safety goggles on and a piece of foreign matter flew into right eye. The supervisor told employee to flush their eye out and seek medical treatment and they refused both saying they would get their spouse to do it at home.
EE and another employee were trying to get a side board off of a trailer. When the other employee was prying it from the tractor trailer it popped off and hit EE in the bottom right leg.

Employee was driving company truck unit #16 and got to Quincy Division on 440 Roberts Sand Rd., and pushed in the clutch with the left foot and the slave cylinder busted and the clutch slammed to the floor and his foot hit the floor.

Employee was operating a front end loader and drove through a water covered hole.
Climbing up spud gantry on dredge and pulled muscle in left thigh.

He was using a grinder on a steel shaft and received a laceration to the top of his left hand when the grinder hit a rough spot and jumped back causing the grinding wheel to cut his hand. Four stitches were necessary.

Employee went to investigate malfunction of dredge and slipped on steps hurting right knee and right hand.
On 3/22/05 EE reported to his supervisor that on 3/14/05 while he was rolling a conveyor belt at the sand plant, his back started hurting. Then he lifted a tool box out of a company pickup truck hurting his back more. Again, on 3/21/04 he was pulling cables to hook up to the dredge, hurting his back more.
