EE was assisting on the ground moving a stacker conveyor. The stacker was being moved with a front end loader. The stacker was moved into position and movement had stopped. EE unhooked chain from front end loader. Stacker started to move, edge of mud flap frame came into contact with right leg of EE causing a laceration which needed stitches.

Employee was stung in both hands by wasps when walking down a set of stairs. Employee's hand went past the open end of a handrail and the wasps had a small nest in the end of the handrail. Insect(Nonvenomous) wasp sting.

Employee stepped back from a piece of machinery and stepped in washout about six inches deep and fell causing fracture right fibula

Employee was removing flashing from cross conveyor. He unbolted the holder and put up on screen deck. Caught middle finger between holder and screen box laceration had 8 stitches to close and a prescription for antibiotics.

On May 23rd, 2012 employee radioed his superintendent that he was having chest pains & trouble breathing. He asked to be taken to the emergency room, which he was. There he was given oral nitroglycerin and morphine. The super stayed until girlfriend arrived. This point on were told by EE this was personal med problem and not WC. On 6/15/12 his attorney told us was work related.

As box was reaching max angle ee felt truck began to lean to drivers side. Immediately began dropping the box. Truck laid over on its driver side door when the box was down to about half way. Hit knee on the door. Went to ER to be checked & returned to work w/o restriction. EE return to DR on 5/31/11 with knee pain. Crutches & immobilizer restriction.
