Employee stepped out of the cab of the truck to clean the mirrors when EE noticed the truck was rolling. While re-entering the cab, EE's left hand was pinched between the cab handrail and the truck bed in front of EE.

The employee was helping install a center pin on a 988 H. EE was using a sledge hammer to check for movement in the pin. On the last swing EE's hand caught a sharp edge on the side of the machine. The injury resulted in a broken tip of the left index finger and a laceration that required nine stitches.

Trackhoe op. called supervisor to exchange trackhoe. Supv walked new trackhoe to the cut. Supv parked new trackhoe beside trackhoe that was in production. Supv was attempting to walk trackhoe off cut & moved tracks in wrong position. Machine walked off 8' cut & overturned trackhoe as a result. Supv slammed rt shoulder in cab w/seatbelt on, breaking shoulder socket.

EE was in process of changing out troughing idler on 30" conv. belt. While performing this task with a co-worker the idler tilted down and came in contact with the conv. frame. EE's right middle finger was mashed between the idler and the conv. frame. EE's right middle finger,last joint recieved approx. 11 stitches.

WHILE ATTEMPTING TO REPLACE COOLENT IN A RADIATOR EE POURED 2 GAL OF ANTIFREEZE OUT OF A 55 GAL DRUM THAT CONTAINED 4.5 GALLONS IN IT. WHEN HE SET THE DRUM UPRIGHT. HE LET IT FALL BACK FROM A 45 DEGREE POSITION HIS RIGHT HAND GOT BETWEEN THIS AND ANOTHER DRUM AND WAS SMASHED. THE CUT REQUIRED 10 STITCHES AND HE WENT BACK TO WORK.

ONE OF THE MEN INSIDE THE CRUSHER REACHED OUT TO PICK UP ONE OF THE WEDGES THAT WAS SITTING ON THE ROTOR SHAFT GU ARD AND KNOCKED THE PART NEXT TO IT OFF WHICH FELL ABOUT 3 FEET AND LANDED ON HIS FOOT CAUSING A 3/4 INCH CUT ON HIS UPPER FOOT THAT REQUIRED THREE STITCHES.
