Driller needed old conveyor belt cut for dust covers. Driller helped cut out an example and instructed employee to cut two more out. While cutting belt with a utility knife with right hand knife slipped off of belt and struck left hand below the thumb.

while changing drill stem on blast drill, pipe wrench would not bite stem. EE was helping it bite the stem. When it did bite, it pinched EE's right hand between wrench and boom mast.

Equipment operator, hauling rock to crusher, in light rain, applied brakes and lost control of CAT 770 off road truck. Laid truck on right side. When seat belt unbuckled, operator fell through right side window, cutting head.
Installing concrete barricades at Quarry entrance. Front end Loader was lifting barricades while removing cribbing from end of barricade cribbing pinched right index finger.

Replacing top screen sections on screen deck on shaker #6 employee was lifting screen section to place it on screen deck. Screen slipped out of employee's hands and fell and came in contact with right middle finger. Employee was taken to ER and received stitches & returned to work with no restrictions from ER doctor. Was instructed to see a orthopedic doctor on 1/23/2017.

The 988 was loading shot rock onto 770 haul trucks. While waiting on trucks to return to pit. Operator move a large rock from loading area the rock slipped out of his bucket causing operator's body to jerk like whiplash. Tightness in the shoulder area. Check by a physician had a slight muscle strain.

Employee's job duties was to help with drilling and blasting operations. Driller had a problem when he was setting up on next drill hole drill pipe disconnected from power head . Employee was attempting to move pipe in pipe rack and pinched his right thumb between two pipes in pipe rack.

EE's job assignment was stripping old floor tiles off of subfloor in q.c. lab. He was using a floor stripping hand chisel. EE was on subfloor and had left arm in front of him for support, the chisel slipped out from under the tile and struck his lower left arm. This resulted in a laceration on lower left arm. No restrictions.

Driving water wagon uphill on ramp from stockpile area to truck scales. Machine lost power, started rolling backwards. Brakes failed, rolled approx 100', jacknifed on level ground. The EE unbuckled his seat belt & jumped or was thrown from cab.

EE was climbing off of side of conveyor when he became entangled in wiring mounted on side of conveyor, causing him to lose his balance and fall appr 4'. EE landed on his hands and knees, causing injury to his left knee.

Loose material sloughed causing rear tires of trailer to drop off stockpile. Trailer turned over on its side causing the road tractor to turn over on its side. The air filter, ferring, mirror, hood were damaged on the road tractor.
Employee was driving a Peterbilt Semi Tractor with a belly dump trailer. Was stockpiling material. EE drove truck through app 6" of water and struck an object which caused the truck to jump, jarring the ee and injuring his back.
EMPLOYEE WAS CUTTING A PIECE OF CONVEYOR BELT, PULLING THE KNIFE TOWARDS HIMSELF WHEN THE KNIFE SLIPPED AND CUT EE'S LEFT LEG ABOVE THE KNEE RESULTING IN AN INJURY REQUIRING (3) STITCHES TO THE SURFACE OF THE WOUND.
