AN EMPLOYEE CLAIMS HE FELL FROM TOP OF THE BUCKET ON A 450 HITACHI WHILE TRYING TO REMOVE A CYLINDER PIN. HE IS CLAIMING HEAD, NECK, AND BACK INJURIES PLUS PERMANENT LOSS OF MEMORY AS A RESULT OF THE FALL. THE FALL WOULD HAVE BEEN APPX 4'6".
