I,NAME WAS FINISHING BOLTING THE SIDES TOGETHER ON THE ROLLER MILL AND HAD JUST DISCONNECTED HIS SAFETY LINE AND WAS GETTING DOWN OFF THE ROLLER MILL WIND BOX WHEN HE SLIPPED AND FELL, THE TOP OF THE WIND BOX IS 4 FT 8 IN. FROM THE FLOOR.
WHILE BAGGING O0F #2 PACKER, THE AIR HOSE THAT IS USED TO BLOW THE MATERIAL LOOSE IN THE BIN CAME OFF. WHEN HE WENT TO SHUT THE VALVE OFF, HE TRIPPED ON A 1" RUBBER HOSE THAT WAS LAYING ON THE FLOOR AND FELL3INTO THE PACKER, HITTING HIS BACK. (LEFT SIDE).

ROLAND BARBER AND GLOVEST SWAIN WERE CHANGING OUT AN EMPTY CHLORINE CYLINDER. THIS CYLINDER IS LOCATED ON THE OUTSIDE WALL OF THE PLANT. GLOVEST DIDNT THINK HE WAS GETTING ENOUGH CHLORINE INTO THICKENER SO HE SHUT THE VALVE OFF AT THE TANK. ROLAND DISCONNECTED THE LINE AT THE PUMP BEFORE THE PRESSURE WAS RELIVED FROM THE LINE. CHLORINE INHALATION CAUSED COUGHING AND AND DIFFICULTY
