EE noticed the forklift EE was operating was running low on propane, so EE decided to replace the almost empty propane cylinder. As EE was putting the empty propane cylinder in the slot, EE's right ring finger was caught in between two propane cylinders and felt a sharp pain on EE's finger. EE ended up with a deep laceration on EE's right ring finger.

One LNA EE was operating forklift and three iTEC EE's were rigging and positioning new MCC section into the Extruder MCC Room. iTEC EE was positioning dolly when rigging slipped off the forklift forks causing contact between MCC section and iTEC EE. iTEC EE sustained injury to right knee.

Hopper chute was plugged with wet material. Process operator opened door in chute that was positioned too high to unplug with prybar. Used heavier an longer bar to unplug chute and heard a pop in EE's shoulder. Immediate pain and numbness in EE's hands and reported to Lead person. Was taken to ER by Manager and then to our doctor same day in afternoon
Employee had a grease gun in a vise, pulling up on it and it came loose from the vise and hit EE between EE's eyebrows which gave EE a cut that needed three stitches.
EE was lifting a flight from a scraper into a loader bucket when the flight slid back and hit EE's left foot right behind the steel toe protector, breaking EE's left toe.

Employee was using a hammer to loosen the inner drill steel on the drilling equipment when the hammer slipped out of their hand and struck employee on the left upper forearm. After being fully released 3 times, On 11/5/2021 employee is scheduled for Physical therapy. At this time appointments are not made yet.

Roller Mill Operator twisted right ankle while operating the bagging station after noticing a torn bag that was traveling up the bag flattener conveyor belt. The Operator exited the bagging station access platform at an angle and when stepping down, missed the second step and twisted right ankle when contacting the floor.
Employee (EE) was walking past an open pallet of empty bulk bags when EE tripped on a length of cordage that was attached to the pallet at one end and loose at the other. The EE had one foot placed on the loose end and the other caught under the cordage causing EE to trip. When EE fell EE impacted the ground with EE's right shoulder.

Operator injured left forearm. The straps on a pallet of empty bags had been cut but not removed completely. EE tripped on the straps and fell forward. EE tried to catch the fall with right arm to the ground and by grabbing the handrail on the nearby steps with left hand. EE missed the railing and left forearm contacted the steps about 3" below the elbow.
A temporary labor contractor slipped and fell on concrete covered with wet clay, landing on the concrete and injuring buttock, hip and lower back.

A CM work order required the mechanic to work on a grease line located inside the body of a CAT Scraper 623E. While leaving the scraper, the mechanic hit his left knee on a metal flight.
The miner was assisting the maintenance department with the removal of a large gear. While using a bar over his head to pry the gear loose, he felt a pain in his lower back.
When the miner was descending from the motor grader, he had both hands on the side vertical railings, then he stepped down with his left foot onto the stirrup step. Then he attempted to place his right foot onto the next step. Missing the second step, the miner swung to his left and fell to the ground. While landing the miner's right foot was tilted inward.

When doing welding work, a spark from the torch came into contact with employee's watch, thus heating the watch up. From the hot watch surface a burn developed on employee's left wrist
It rained much of the day on 5/6/14 and since IMV is a clay mine, it becomes very slick after rain. employee was climbing a ladder that was covered in clay and slipped. He caught himself though, but scraped and bruised up his knees
It was very windy on 4/25/14. While stacking bags onto a pallet, clay was blown into ee eye and after rubbing and trying to clean it out, he decided to tell a supervisor

The morning of 9/20/2012 as employee was descending from his bulldozer he felt a sharp pain in his right foot. It is believed to be from a combination of descending too fast and him getting his foot twisted and stuck in the ladder while descending.
Employee hurt his right shoulder as he tried squeezing through a vertical pipe and bagging machine hitting his shoulder on an air line.

employee was cleaning work area when he lifted a 50 pound bag straining his lower back to the point where he could not walk with out being assisted. He was taken to pahrump nv to the hospital.
Miner was blowing debris with air hose when a piece of debris fell in his right eye. Miner did not report injury until 8/4/11 and was taken by Safety to see an eye specialist. Doctor did not find anything wrong with eye. Said the red will go away in a few days. Miner returned to work and finished his day. Miner was wearing his safety glasses.

Miner was working in diesel shop along with another miner, 2.5# hand sledge hammer slipped from ee grasp and impacted miner between eyes, handle first. Miner thought injury was superficial, but sought medical attention on 10/15/10 due to dizziness, blurred vision in left eye,ringing in left ear, and headaches.
Wind blew dust around safety glasses and into left eye.
Miner was operating ore truck when he began feeling dizzy and disoriented. Miner pulled truck to side of road, but struck a small bush resulting in slight damage to passenger side of tractor. Miner says he never completely lost consciousness. When Miner exited vehicle to survey damage, he slumped to the ground scraping his left arm, as he had no strength in arms or legs.

Miner caught left "pinkie" finger in door of extruder. Informed company on 09/09, but said injury was minor. On 09/14 management set up appointment with local clinic for 09/15. Miner returned to work with splint.
09:15 AM miner reached under a pallet to retrieve an empty 50# bag. An unidentified animal bit miner on right index finger. Miner reported accident to General Manager at 10:20 AM. Miner taken to Amargosa Clinic at 10:40 AM, treated and released.

Miner was operating palletizer. Bagging chair is equipped with seat belt safety switch. Safety will not allow conveyor belt to operate unless miner has seat belt fastened. Miner removed seat belt, conveyor stopped, and miner stepped onto belt. Unknown to miner, female end of seat belt contained clay which allowed conveyor to restart. Miner fell on right hip.
Miner was stepping down off fork truck and twisted left ankle. There was no swelling and little pain at time. Miner completed shift. Over next few days, ankle continued to bother Miner. Visited doctor on 8/22 and 8/25/2008. Off work on 8/22 only. Diagnosed as sprain.
Wind blew dust around safety glasses into right eye.
Miner was bitten on tip of right index finger by small sidewinder snake.
Miner was operating automatic palletizer. Right foot was caught in pneumatic doors. Steel toes in safety shoes held doors open until pressure could be relieved and doors opened. Miner suffered abrasions on both sides of right ankle and possible hairline fracture.
Miner slipped on wet clay from recent rain.
Miner states he injured left knee while climbing stairs. Did not notify management until 02/05/2008. Injury disputed by Workman's Compensation due to pre-existing problems with knee. Claim upheld on 04/14/2008 and IMV Nevada notified on 04/21/2008. Accident Report filed 04/21/2008.
