SOLID ROCK CRUSHING, LLC · Struck by falling object

IN LOTO status, EE #1 was removing the motor box cap from the conveyor 10-12 ft above ground. Two others were on the ground observing EE #1. EE #1 dropped a bolt from above and EE #2 stepped into work area to retrieve the bolt. The cap dropped just as EE #23 was exiting the area and was struck by cap on their left hand/wrist. EE #2 was wearing a hard hat.
