EMPLOYEE WAS FEEDING TH EWASH PLANT PART OF PLANT WHICH STOPPED, EMPLOYEE WEN TOT SHUT PLANT DOWN AND SLIPPED OFF OF LOADER NAD FELL TO GROUND. EMPLOYEE ON CAT-WALK OF LOADER.
EE AND EEER WERE REMOVING TONGUE BOLTS ON CONVEYOR. THE EE REMOVED THE TWO BOLTS ON HIS SIDE. THE EE WAS THEN TO REMOVE THE TWO BOLTS ON HIS SIDE WITH A HAMMER AND PUNCH. WHEN THE EE GOT TO TH E LAST BOLT HE TOLD EE TO STAND BACK AND LET TONGUE DROP 4 INCHES INTO LOADER BUCKET. THE EE REACHED FOR TONGUE AND CAUGHT HIS FINGER BETWEEN TONGUE AND LOADER BUCKET.
