Arepet Industries, LLC · Fall from machine

Employee was closing the hatches on a pneumatic truck. While attempting to get back onto the catwalk employee fell off the truck. Employee was wearing EE's harness but EE did not tie off properly. Emergency responders attempted to render first aid but were unsuccessful. At this moment I do not have a detailed description of all the contributing factors that led to the fall.

Employee was performing routine cleaning of clogged screens in the dry plant due to wet sand buildup. While inspecting for a blockage, the employee opened the diverter access door. Accumulated hot sand was released from the chute area and contacted the employee, resulting in burns requiring medical treatment.

The EE was cleaning the #1 Baghouse and placed EE's right hand into the rotary airlock. This resulted in lacerations to EE's right fingers and a fractured right pinky.

The contract plumber was attempting to install a section of pipe. The plumber was on their company provided ladder, when it appeared to collapse causing the plumber to fall and fracture their left leg.

The EE was shoveling sand under the wet conveyor belt and felt a pain to lower back. The EE was placed on restriction for the injury.

While on break a plant helper went to car to rest. A few minutes later while adjusting the car seat the employee accidentally discharged their firearm. This resulted in a hand injury due to a gun shot wound. Employee was terminated due to violation of Arepet gun policy which is mentioned in the employee safety handbook.

Employee was on the cat walk while attempting to guide a customer truck into position. The miner squatted down to make visual contact and felt pain in lower back as they stood up.
