After safety check, operator turned the screening plant motor on to warm up. After 2-4 minutes of the motor running, the operator noticed a white vapor coming from the hydraulic pump. Operator went to shut the screening plant down. When the operator attempted to turn the key off (shutting down the plant) the hose came off the pump, resulting in hydraulic oil spraying the operator.

EE was attempting to remove a rock from between 2 rear tires when the insode tire blew open. The EE received a fractured R. wrist, fractured index finer on R. hand., a laceration to his R. thumb (requiring 7 stitches) and a small cut to his forehead, EE was taken to ER and realeased around 2200 that night.
