The employee was picking up a 50 lb bag of material to pour into mixing hopper. The employee felt some pain and heard a pop in their lower back area.

Employee was kneeling on top of a double stacked pallet, forklift driver was down stacking pallets to be identified by the tag on the ears. Employee kneeling on the double stacked pallet on the next row reached for the ear and fell forward finger was damaged on the fork.

Two employees working closely beside each other, one employee cut the other with a box cutter during horseplay.

Employee was complaining about legs hurting, supervisor then brought EE into office to cool down. After resting for a few minutes EE was still complaining about pain in the legs. Management decided to take EE to BHP to be checked out. To go shower out, EE fell down the stairs. Supervisor ran out to check on EE and called for management to call ambulance.

Employee walking beside railroad track tripped on rope tied to guardrail. EE stumbled but caught balance. EE reported no injury at that time and was able to finish the rest of the shift. After a day or so EE's foot started swelling so we sent EE for X-Ray.

The employee was in process of filling propane bottles and placing them on the storage rack. Employee went to place a full bottle on the rack and inadvertently caught left pointer Finger between the bottle and the rack. As a result smashing the finger and causing a laceration.

employee was opening a unloading door on the bottom side of a hopper car. EE was using the tanker mans bar and when EE went to open it, the position EE was in caught a pinch in EE's back.

Employee was unloading a drum truck. As the employee was unloading the empty 55 gallon metal drums, they made it to the last row and the employee was pulling a drum forward and it slipped out of EE's hand, rocking back and smashing EE's thumb on the right hand.

Employee was running a bead on a finished product bin and got a piece of metal in eye. Employee had on a hood, along with cap and welding gloves.

Employee was cutting a gum rubber boot with a folding knife and no cut gloves on. The employee was cutting in a upward motion and knife blade turned in EE's hand, at this point the knife blade closed on EE's finger.

Employee was pulling bags down off a double stacked pallet. The double stacked pallet height reach 65 inches in height. As the employee was pulling the bags off EE felt a pop, and EE's shoulder went to hurting. (right shoulder) The individual bag weight is 50Lbs per. The employee was mixing a batch mix and did not wait on the forklift to come.

Employee had an energy drink which caused EE to feel like EE couldn't breathe and EE's heart to race. Foreman took EE outside to get fresh air and provided EE with water. Plant manager was notified and EE's pulse and blood pressure was taken. An ambulance was called and it was determined that the employee was having an anxiety attack.

Employee was replacing a bearing on ACM shaft. EE adjusted the height of the base plate for bearing. EE placed bearing and shaft into position. EE stuck EE's right middle finger into alignment hole to check position, the base plate slipped and pinned EE's finger. EE received a laceration on tip of middle finger which required stitches. Press was pulled from service.

Employee threw a 50 lb bag over the bag press at Jet Mill 2 and strained EE's back/neck. The employee was taken to local ER where a CT scan was obtained and showed two bulging discs in EE's neck. This could have been from a previous auto accident prior to EE's employment here. This was turned over to workman's comp and EE will be given an MRI.

Employee was tempting to open the bottom door of a rail car to unload material. Safety latch was stuck, when safety latch released employees finger was pitched causing tip of finger to burst. Employee was taken to emergency room and got 5 stitches.

Pulling the tin off of the south side of the building. Employee tied onto the tin with a rope so as to have more control of tin. While trying to tie the tin it started to fall and employee cut his finger.

Transporting a 1" thick steel plate with forklift. He parked the forklift next to the stands that was going to hold up the sheet. He then grabbed the sheet and started to slide it off the forks on to the support stands. The pulling motion caused stiffness and pain in his mid back region.

Employee was found unresponsive on the Rollermill #1 3rd level. The employee had been seen just two minutes before and showed no indication of a health problem.

Employee came in contact with a welding lead (stinger) that was hanging in the work area. The employee was shocked when he contacted the lead (stinger) and a grounded piece of equipment.

Employee was in the process of changing screens. He noticed that the screen appeared to be pulled more to one side. He reached his hand into the front of the shaker to check if it was still hooked properly. At the same moment the back side of the screen came off and the screen recoiled. The recoil pinned his right hand between the front hook point and the shaker wall.

Employee was attempting to straighten a 50lb packer that was sitting on a pallet that had shifted. The employee jerked on the leg of the packer and pulled a muscle in his mid back.

Employee was checking the railcar and was sitting on the plant floor with railcar directly in front on him. He jumped to the ground using his arms to support him. When he jumped down, he landed on his right ankle and knee.

Employee was working on a piece of equipment on his knees in the maintenance shop.

Employee was on forklift backing down ramp after dumping a bag of feed in the Rollermill. The forks were extended up with the empty sack on the forks. He went down the ramp at an angle catching the corner of the ramp. This caused the forklift to tilt over. He was complaining of chest, back, and left thigh pain. He also said right eye was blurry.

Twisted right ankle as employee was climbing down the extention ladder, about 4 rungs from the bottom, when his foot rolled. Ladder was reported as being wet.

Employee was putting Socks on Bag House, when he slipped off the rails. He straddled one of the bars which are about 2"x2" wide. He landed on his scrotum area.

Employee was cutting the bottom out of a supersack of raw material. The bag bulged out when cut & pinned his left arm & shoulder between the hopper and bottom of sack. He then jerked his arm out of the pinch point.

Employee was placing .sprocket on the elevator shaft on the roller mill. His knee came in contact the back elevator's door flange. This caused pain and swelling in the right knee.

Employee was working on the main chain on a forklift mast. He stood on the dash board of the forklift with his body positioned between the roll cage and mast. While he was working, his foot engaged the tilt back lever pinning him against the roll over cage. A co-worker saw what happened and tilted the mast back to free him.

Employee slipped on stairs hurting his left ankle. Stairs were slippery with graphite.

Employee was picking up 25 lb. boxes & started having pain and popping sensations in his left upper back at times. He reported that he felt like he was having muscle spasms.

Employee was rolling up an air hose and he stepped on the connector causing him to twist his left foot and knee outward. Employee had pain and swelling in the left knee.

Coworker used threading machine to cut threads on pipe. EE was holding backup pipe with a pipe wrench so coworker could remove thread cutter. EE accidentally stepped on the On/Off switch, engaging machine. EE was pulled up & over rear of machine; fingers pinched between wrench and machine housing. Amputation distal phalange R. ring finger & broken L. index finger - severe inj.

Employee was stepping up onto a platform when he stepped on a bag on the ground. He thought the bag was on the cement but there was nothing under the bag. This caused him to twist his right knee. This was reported on Jan. 4th to the Safety Director. Employee come back to Safety Director on Jan 10th complaining that his lower back, neck, and shoulder was hurting.

Employee was throwing 50 lb. bags on a pallet. Right hand got hung in the bag and the hand rolled causing his arm to roll.

Employee was walking around blender to check his reader. Employee's feet slipped out from under him causing him to fall and land on his right knee.

Employee was reaching overhead to unhook bags. The left side of the middle of his back began to feel tight. (like a muscle spasm)

Employee was filling 35 lb. 5 gallon buckets with material. He had pain in his lower right side of his abdomen.

EE WAS CUTTING LIDS OFF OF PLASTIC 5 GAL. BUCKETS WITH A BOX CUTTER. HE CUT HIS HAND WITH BOX CUTTER. (CUT 2 TENDONS IN HIS LFT. HAND).

EE WAS ON FORKLIFT BACKING DOWN A RAMP & HIS BACK TIRE CAUGHT THE EDGE OF THE RAMP CAUSING HIM TO FLIP THE FORKLIFT OFF THE RAMP. THE FORKLIFT LANDED ON ITS SIDE. EE WAS WEARING SEATBELT.

EMPLOYEE HAD A PIECE OF SLAG IN HIS EYE CAUSING IRRITATION. IT IS UNKNOWN HOW IT HAPPENED.

EE WAS BAGGING MATERIAL WHEN HE GOT OVER HEATED HE STARTED VOMITING & HAVING STOMACH CRAMPS. IT WAS HOT & HUMID & EE DID NOT DRINK ENOUGH WATER. ALSO, SHORTNESS OF BREATH

EE WAS SHOVELING UP GRAPHITE IN ACM ROOM WHEN HEGOT GRAPHITE DUST IN HIS EYES CAUSING THEM TO BECOME IRRITATED

EE WAS USING CRESCENT WRENCH TO TIGHTEN A NUT & BOLT & EE STRAINED ELBOW

EE WAS ASSISTING CO WORKER IN RETRIEVING PART FROM TOOL ROOM WHEN A STEEL PLATE FELL OFF SHELFCUTTING HIS FOREHEAD

FINISHED STACKING PALLET OF CPC60 AND GOT OFF OFFORKLIFT AND MOVED PALLET. CAME BACK TO WORK AREA. STEPPED OFF FORKLIFT AND FELT SHARP PAIN IN LOWER STOMACH.
