Blt Sand & Gravel Inc · Caught in, under or between two or more moving objects

LOADER OPERATOR ATTEMPTED TO CLEAN DIRT FROM TAIL PULLEY OF DRY SCREEN PLANT WHILE PLANT WAS RUNNING. HIS ARM WAS CAUGHT IN PULLEY. HE BROKE HIS ARM AND RECEIVED FRICTION BURNS ON HIS ARM AND CHEST AREA. HE WAS A NEW EE AND HAD BEEN INSTRUCTED BY PLANT FOREMAN AND OTHER OPERATORS NOT TO ATTEMPT ANY REPAIRS BUT REPORT ANY PROBLEMS TO CO-WORKERS.
