Aztec Materials, LLC · Fall to the walkway or working surface

Employee parked at office trailer and proceeded to walk to the shop by taking the shortest route, straight under the conveyor of the stone plant. Plant had wet clay due to active use of plant. Employee fell on wet clay causing injury to leg. Lights were on and sun was up and no ice was present.

Employee was removing a temperature sensor form the rear axle of a wheel loader. Employee tried to back the sensor out just enough to bleed out any accumulated pressure, but went too far and right hand was sprayed with hot oil. First and second degree burns to palm and wrist.
