EMPLOYEE WAS LIFTING A WET BRAKE OUT OF THE RAIL RUNNER WHEN HE CUT HIS RIGHT HAND FROM A PIECE OF METAL ON THE BRAKE. HE WAS TRANSPORTED TO THE HOSPITAL AND RECEIVED 4 STITCHES TO HIS HAND.

EE STATED THAT HE WAS PUTTING SUPPLIES ON THE ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL AND STRUCK HIM BEHIND THE RIGHT EAR. THE INJURY CAUSED A LACERATION BEHIND THE EAR REQUIRING 13 STITCHES. EE MISSED WORK ON 11-19 AND 11-20 THEN RETURNED TO WORK ON 11-22-04.

Ee stated that he was using a slate bar to lift a belt rope when the bar slipped and struck him in the ribs. He continued to work until 6-22-04 when he consulted a Dr. and it was determined that he was suffering from bruised ribs.

EE WAS USING A HAMMER TO CHIP A CONCRETE BLOCK TO SIZE WHEN HE STRUCK HIS LEFT HAND. HE WAS TRANSPORTED TO THE HOSPITAL AND RECEIVED 2 STITCHES AND RETURNED TO WORK THE SAME DAY.

EE STATED THAT HE WAS HELPING THE ROOF BOLTER CHANGE PLACES WHEN THE REEL WAS ENGAGED AND CAUSED THE CABLE TO SWEEP EE'S FEET FROM UNDER HIM CAUSING HIM TO FALL ON HIS HIP AND LEGS. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS XRAYED AND TREATED FOR A MUSCLE SPASM.

EMPLOYEE STATES THAT A PIECE OF ROCK APPROXIMATELY 8' X 3' X 3" FELL FROM BETWEEN ROOF BOLTS AND STRUCK HIS LEFT HAND. HE WENT TO THE E.R. AFTER WORK WHERE X-RAYS WERE NEGATIVE.
