Couch Aggregates LLC · Fall to the walkway or working surface

The operator of the Volvo EC380 Excavator was on the ground fueling the machine with a cord (24v-used to fuel machine as needed) and the male end was not covered to prevent contact with metal. The male end came in contact with the metal frame causing a arc flash. The operator was scared, jumped back and tripped falling to the ground.

While wearing safety glasses, employee was working around grinding, cutting and welding during the work day, causing dry irritated eyes. Medical treatment by diagnostic eye exam and prescription eye drops for employee.
