Baker Hughes Oilfield Operations LLC · Fall to the walkway or working surface

Operator was walking to the scale to check the reading on the scale, heard a noise coming from building, turned quickly and fell. EE did not slip or trip on anything. The area EE fell had no hazards that contributed to the fall. The conclusion is that when EE turned, EE lost balance and fell.

Removing busted bag from between conveyor belt and roller table. When attempt to grab bag glove caught conveyor belt, pulling right hand between belt and roller causing hand to be wedge between.

Maintenance man cut grating section from crushing area. Left for lunch after lunch went back to crushing area to finish job and fell through section that was cut out.

EE left his tools on barge he was loading earlier. Went down the gangway to pick up tools and a tug with 6 barges ran into dock and barge he was on shifted he fell to barge. Right knee started to swell that morning. 2 weeks later he was starting to load another barge & picked up on gangway to put on barge when his knee failed & he fell to the ground. Happened on 8/10/11.

Forklift op was backing out of the bagging room and struck on of the contract EE's that was standing near overhead door with the forklift. The EE fell down and the forklift ran over his leg. The EE was taken to local ER for treatment and released. X-rays were negative and he had no fractures. He received presciption medication for pain.

Employee went up on top of crusher to help unplug it. He put the basket up along side of handrail. When he got out of the basket it popped up and pinched his index finger between the handrail on crusher and basket of manlift.

PREPARING TO LOAD BARGE, HE STEPPED ON GRATING WALKWAY WITH LEFT FOOT. AS HE WAS PUTTING RIGHT FOOT ON GRATING, THE LEFT FOOT SLIPPED OFF CAUSING HIM TO FALL FORWARD. HE PUT OUT HIS HANDS SO HE WOULDN'T HIT HIS FACE ON THE WALKWAY, CUTTING HIS HANDS.

HE WAS CHANGING SHAFTS AND HANGER BEARINGS ON #2 SCREW. TOOK OUT BOLTS THAT HOLD SHAFT TO SCREW. PUT PIPE WRENCH ON SHAFT THEN TURNED ON MOTOR TO BREAK SHAFT LOSE FROM SCREW. THE SCREW CAME OU T OF PAN HITTING HIM IN THE RIGHT ARM AND PUSHED HIM INTO GUARD RAIL, HITTING HIS LEFT SIDE.
