While cutting tube with a box knife the driller cut self with the knife and EE left the quarry without telling anyone what happened. Once EE arrived home, EE realized it was more than EE could take care of so drove self to a medical facility receiving 4 stitches to inner calf on EE's right leg. EE was released back to work with no restrictions.

While changing a drill bit, the wrench would not grip the drill hammer. Using a 3lb sledgehammer to tighten the wrench, the employee swung the hammer which ricocheted off the frame of the boom. Which caused EE to strike left hand that was holding the wrench.

The employee was attaching a tooth on the bucket of 990 loader. When striking the locking pin on the loader tooth, a piece of metal broke off and penetrated the employees front upper thigh.

EE was working on concrete forms. He was driving concrete forming stakes with a jack hammer. When pulling the jack hammer off the stake he tripped & pinched his finger between the jack hammer handle & a concrete stake. He was sent to the hospital for evaluating. He had a cut to the finger requiring stitches & bandaging.

LIFTING A 300 LB METAL BOX TO BE INSTALLED IN CHUTE WITH BACK HOE BUCKET & CHAIN. BOX WAS NOT HANGING STRAIGHT AFTER PICKING IT UP 3 FT., ATTEMPTED TO STRAIGHTEN BOX WHEN THE BACK END CAME LOO SE, BOX SLID DOWN LEG & STRUCK TOP OF RIGHT FOOT, BREAKING 1 BONE & CHIPPING ANOTHER.
